Zusammenfassung
Anamnese und klinischer Befund: Ein 64jähriger adipöser Mann klagte über ein nicht belastungsabhängiges retrosternales Druckgefühl mit Ausstrahlung in Hals und Rücken. Die körperliche Untersuchung ergab keine richtungweisenden pathologischen Befunde. Der Patient war zum Ausschluß einer koronaren Herzkrankheit vor 15 Jahren koronarangiographiert worden, ohne daß eine Ursache für die beschriebenen Beschwerden gefunden wurde. Damals zeigte sich im Ruhe-Elektrokardiogram eine diskrete T-Wellen-Negativierung linkspräkordial.
Untersuchungen: Im Elektrokardiogramm fielen jetzt neu terminal tief negative T-Wellen linkspräkordial und in den Extremitätenableitungen sowie ein positiver Sokolow-Lyon-Index von 4,8 mV auf. Im enddiastolischen Standbild der Ventrikulographie ließ sich eine zirkumskripte, rein apikale linksventrikuläre Hypertrophie mit typischer »ace-of-spade«-Konfiguration nachweisen.
Diagnose, Therapie und Verlauf: Anhand des Ergebnisses der kathetertechnischen Untersuchung wurde die Diagnose einer umschriebenen apikalen Linksherzhypertrophie gestellt. Eine spezifische Therapie erfolgte bei nur geringer Beschwerdesymptomatik nicht.
Folgerung: Die umschriebene apikale Linksherzhypertrophie kann im Langzeitverlauf eine erhebliche dynamische Entwicklung erfahren. Bei ausgeprägter T-Negativierung sollte, auch bei vormals unauffälligem Herzkatheterismus, an eine umschriebene apikale Linksherzhypertrophie gedacht werden. Patienten mit atypischer Angina-pectoris-Symptomatik, zunehmenden richtungsweisenden EKG-Veränderungen und vormals unauffälliger Koronarangiographie sollten in 2-3jährigen Abständen transthorakal echokardiographisch, unter besonderer Beobachtung der Herzspitze und unter Verwendung höherfrequenter Schallgeber, untersucht werden.
Abstract
History and clinical findings: A 64-year-old obese man had for 15 years suffered from exercise-independent retrosternal pressure sensation, radiating to the neck and back. Shortly after the onset of these symptoms he had undergone coronary angiography with negative results. But at that time the resting ECG showed discrete T wave negativity in the left precordial leads.
Investigations: At the present admission the ECG showed deeply inverted T waves in the left precordial and limb leads and a positive Sokolow-Lyon index of 4.8 mV. Left ventricular angiography demonstrated in enddiastole a circumscribed myocardial hypertrophy limited to the apex and of typical »ace of spade« shape.
Diagnosis, treatment and course: Left-heart catheterization and angiocardiography provided the diagnosis of circumscribed apical left ventricular hypertrophy (ALVH). As the patient had only minor symptoms no treatment was given.
Conclusion: Circumscribed ALVH can show marked dynamic development in long-term observations. If there is marked T wave negativity, even with previously normal LV angiography, circumscribed ALVH should be included in the differential diagnosis. Patients with atypical angina pectoris and increasingly suggestive ECG changes should, even if previous coronary angiography had been negative, undergo transthoracic echocardiography with a high-frequency transducer, special attention being paid to muscular changes at the LV apex.
