Diagnose und Therapie eines intramuralen Hämatoms der Aorta ascendens

Zusammenfassung:

Anamnese und klinischer Befund: Bei einer 79-jährigen Patientin entwickelten sich aus völligem Wohlbefinden heraus starke thorakale Schmerzen und Dyspnoe.

Untersuchungen: Die Röntgen-Thorax-Aufnahme zeigte ein verbreitertes Mediastinum und einen kleinen Pleuraerguss links. Bei der transösophagealen Echokardiographie fand sich eine Erweiterung der Aorta ascendens und eine Aorteninsuffizienz. Ferner fiel ein inhomogener echoarmer Saum in der Aortenwand auf. Diese Struktur wurde in der Computertomographie als intramurales Hämatom interpretiert.

Therapie und Verlauf: Dieser Verdacht wurde in der sofort durchgeführten Aortenersatzoperation bestätigt.

Folgerung: Das Intramurale Hämatom der Aorta ascendens ist mitunter schwierig zu diagnostizieren, stellt jedoch einen sofort operationsbedürftigen Notfall dar.

Abstract

History and clinical findings: A 79 year-old female presented with severe chest pain and shortness of breath.

Investigations: The chest X-ray showed a widened mediastinum and a small leftsided pleural effusion. Transesophageal echocardiography demonstrated not only dilatation of the ascending aorta and aortic regurgitation but also an echo-poor crescentic seam within the aortic wall. Computed tomography identified this structure as an intramural hematoma.

Treatment and course: The aortic valve and the ascending aorta were replaced.

Conclusion: An intramural hematoma of the ascending aorta may be difficult to recognize but it is a life-threatening condition and should be treated by prompt surgical repair.
