Pathophysiologie der Entzündung und ernährungsmedizinische Intervention

Zusammenfassung

Die Prävalenz der Autoimmunerkrankungen steigt in den Industrienationen während der letzten Jahre ständig an. Pathophysiologisch sind Autoimmunerkrankungen durch eine gesteigerte Entzündungsreaktion gekennzeichnet, die infolge des persistierenden Antigens perpetuiert. Proinflammatorische Mediatoren, die als Cytokine, Chemokine und Eicosanoide miteinander interagieren, werden überschießend gebildet. Die Forschung der letzten 20 Jahre hat gezeigt, dass das Eicosanoidsystem ernährungsabhängig beeinflusst werden kann. Arachidonsäure wird mit der in Industrienationen üblichen hohen Fleischzufuhr exzessiv zugeführt und ist die Ausgangssubstanz proinflammatorischer Eicosanoide. Eicosapentaensäure und die aus ihr gebildeten Eicosanoide wirken entzündungshemmend. Wegen des geringen Fischverzehrs überwiegen in den Industrienationen die proinflammatorischen Stimuli. Möglicherweise ist dies auch ein Grund für das gehäufte Auftreten der Autoimmunerkrankungen und anderer Erkrankungen mit gesteigerter Eicosanoidbiosynthese in den Industrienationen.

Abstract

The prevalence of autoimmune diseases in the western world has increased continuously during the last years. The common pathophysiological feature of autoimmune diseases is an exaggerated inflammatory reaction, which is perpetuated by a persisting antigen. Proinflammatory mediators, e.g., cytokines, chemokines and eicosanoids mutually react with one another and are formed in abundant amounts. Research of the last 20 years has revealed definitive dietary means to attenuate eicosanoid biosynthesis. Arachidonic acid is the precursor of proinflammatory eicosanoids, and its intake is high with the ample meat consumption in the industrialised countries. Eicosapentaenoic acid and eicosanoids derived from it show anti-inflammatory properties. Because of the low fish intake, pro-inflammatory stimuli prevail in the industrialised countries. This could be of relevance for the increasing prevalence of diseases with exaggerated eicosanoid biosynthesis in the industrialised countries.
