Anamnese und klinischer Befund: Wir berichten über einen 66-jährigen Patienten, bei dem im Alter von 65 Jahren nach einer pathologischen Fraktur des Femurs erstmals ein medulläres Plasmozytom diagnostiziert worden war. Bei einer erneuten Aufnahme wegen Lumboischialgien kam es zu einer zunehmenden Verschlechterung des Allgemeinzustandes mit abdominellen Beschwerden und septischen Laborkonstellationen.
Untersuchungen: Die klinische, sonographische und computertomographische Untersuchung ergab die Verdachtsdiagnose einer akuten Cholezystitis.
Therapie und Verlauf: Es wurde eine Cholezystektomie durchgeführt. Die histopathologische Begutachtung der Gallenblase ergab eine metastatische Infiltration durch das medulläre Plasmozytom. In den meisten Fällen bilden sekundäre Tumoren der Gallenblase nur wenige Millimeter große Herde. Der vorliegende Fall zeigt eine bis jetzt noch nicht bekannte, die gesamte Gallenblasenwand betreffende Ausdehnung.
Folgerungen: Sekundäre Tumoren der Gallenblase werden als selten angesehen. Seit 1863 wurden 95 gesicherte Fälle beschrieben. Die vorliegende Kasuistik beschreibt den zweiten Fall eines in die Gallenblase metastasierten medullären Plasmozytoms. Die aktuelle Zusammenstellung publizierter und gesicherter Fälle lässt vermuten, dass sekundäre Tumoren der Gallenblase durchaus häufiger vorkommen als angenommen. Deshalb sollten die sekundären Gallenblasentumoren in der Differenzialdiagnostik einer akuten Cholezystitis berücksichtigt werden.
Metastasis of medullary plasmozytoma as cause of acute cholecystitis
History and admission findings: A 66-year-old man was found to have a medullary plasmocytoma the year before admission, after sustaining a pathological fracture of the femur. During treatment for sciatica his general condition deteriorated and he developed abdominal pain. Laboratory tests suggested a septic process.
Investigations: The clinical and sonographic findings and computed tomography suggested an acute cholecystitis.
Treatment and course: A cholecystectomy was performed. Miscroscopic examination revealed metastatic infiltration of the gall bladder by a medullary plasmocytoma.
Conclusion: Secondary tumours of the gallbladder considered are thought to be rare. Since 1863 only 95 cases have been reported. The case discussed here is only the second one in which the metastatic tumor was a medullary plasmocytoma. But a search for published and confirmed cases indicates that secondary tumors of the gall bladder are not so rare. Metastatic tumors of the gall bladder should be kept in mind in the differential diagnosis of acute cholecystitis, especially in patients with an advanced primary tumor.
