Zusammenfassung
Hintergrund: Neueste Entwicklungen und veränderte Indikationsstellungen der Prothetik bei der oberen Sprunggelenkarthrose erfordern angepasste physiotherapeutische Nachbehandlungsrichtlinien. In der Literatur existieren gegenwärtig weder physiotherapeutische Richtlinien noch Outcomes einer postoperativen Rehabilitation.
Ziel: Das Ziel dieses Fallberichts bestand darin, anhand eines Patientenbeispiels Richtlinien eines klinisch bewährten 4-Phasen-Rehabilitationsprogramms nach der Implantation einer oberen Sprunggelenkprothese zu erläutern.
Methoden: Ein 59-jähriger Mann durchlief ein 4-Phasen-Rehabilitationsprogramm mit entsprechender individueller Anpassung der Therapiemaßnahmen. Die klinischen und funktionellen Outcomes wurden bis zu 1 Jahr postoperativ verfolgt.
Ergebnisse: Am Ende des 3. postoperativen Monats war der Patient fast schmerzfrei (VAS:1 / 10), erreichte die intraoperative Beweglichkeit (PF/DE: 30 / 0 / 10) und konnte allen Alltagsaktivitäten nachgehen. Bezüglich sportlicher Aktivitäten konnte er wieder Ski fahren (6. Monat) und Golf spielen (10. Monat). Während der Rehabilitation traten keine Komplikationen auf. Die neuromuskuläre Kontrolle des Fußes und der Beinachse verbesserte sich bis zu 1 Jahr postoperativ.
Schlussfolgerungen: In diesem Fallbeispiel begünstigte das Befolgen eines 4-Phasen-Rehabilitationsprogramms das Erreichen symptomfreier Alltags- und Freizeitaktivitäten. Klinische Studien bestätigen das Resultat, jedoch muss die wissenschaftliche Wirksamkeit des Programms weiter untersucht werden.
Abstract
Background: Latest developments and changed indications in prosthetics of ankle joint arthrosis require adjusted physiotherapeutic follow-up treatment guidelines. So far there exist neither physiotherapeutic guidelines nor outcomes after postoperative rehabilitation.
Objective: The aim of this case study was to illustrate rehabilitation guidelines after the implantation of an ankle joint prosthesis by means of a clinical approved 4-phases rehabilitation programme.
Methods: A 59 year-old male went through a 4-phase rehabilitation programme with appropriate individual adjustments regarding therapeutic measures. The clinical and functional outcomes were monitored postoperatively up to 1 year.
Results: At the end of the third postoperative month the patient was almost analgised (VAS: 1 / 10), had reached intra-operative mobility (PF/DE: 30 / 0 / 10), and could follow all his every day activities. As to sports, he could ski (6th month) and play golf (10th month) again. During rehabilitation there occurred no complications. Neuromuscular control of the foot and the leg axis improved up to 1 year post-operative.
Conclusion: In this case study a 4-phase rehabilitation programme encouraged the achievement of symptom-free everyday and leisure activities. Clinical studies confirmed the result but the scientific effectiveness of the programme needs further research.
