Ganganalyse und physiotherapeutische Behandlung bei einem Patienten mit Stickler-Syndrom

Zusammenfassung

Bei Patienten mit Coxitis oder Perthes-like Symptomatik der Hüftgelenke und Beteiligung der Augen (Ablatio retinae) sollte differenzialdiagnostisch an ein Stickler-Syndrom gedacht werden. Bei einem 12-jährigen Patienten mit Stickler-Syndrom und Hüftbeteiligung wurde eine ausführliche Ganganalyse zur weiteren Abklärung der Hüftproblematik und deren Auswirkungen auf das gesamte Skelettsystem durchgeführt. Dabei zeigte sich ein Beckenhochstand rechts, bei gleichzeitiger Abduktionsstellung der linken Hüfte sowie bds. eine sehr starke Beckenrotation. Neben der Schmerztherapie verbesserte die intensive Physiotherapie die Funktion der Hüfte, sodass die geplante Operation der Hüfte (laterales Release der Hüftkapsel) vorerst umgangen werden konnte.

Abstract

In patients with coxitis or Perthes-like symptoms of the hip combined with ocular symptoms (e. g. ablatio retinae) one should think of Stickler syndrome. This disease is also known as hereditary arthro-ophthalmopathy. It has autosomal dominant inheritance. In 75 % of the patients with Stickler syndrome mutations of the COL2A1 gene located on chromosome 12q13.11-q13.2 have been identifed. These mutations result in abnormal production of type II collagen, a major component of cartilage, vitreous body, and nucleus pulposus. Other mutations in patients with Stickler syndrome are in the COL11A1 und COL11A2 genes. There is a wide variety of symptoms. The disease may affect the eyes, ears, heart, and musculoskeletal system. Retinal detachment occurs frequently. We performed a gait analysis on a 12-year-old patient with Stickler syndrome and Perthes-like symptoms in order to illustrate the effect of hip involvement on the whole skeletal system. It revealed a pelvic elevation on the right with an abduction position of the left hip as well as bilateral severe pelvic rotation. Intensive physiotherapy together with adequate drug treatment for pain relief is important to improve joint function. This therapy helps to prevent early surgery.
