Zusammenfassung

Einleitung: Das Ziel der vorliegenden Studie bestand in der Evaluation der intraoperativen Technik sowie die funktionellen und reintegrativen Ergebnisse nach elastischer stabiler intramedullärer Nagelung (ESIN) von dislozierten Klavikulaschaftfrakturen. Material und Methoden: Zwischen 2003 und 2006 wurden 15 Patienten mit dislozierten Klavikulaschaftfrakturen mittels ESIN behandelt. Wir untersuchten die funktionellen Ergebnisse und Komplikationen. Ergebnisse: Der mittlere Constant-Score betrug 95,3 ± 3,9 Punkte, die mittlere Arbeitsunfähigkeit 22 ± 9 Tage. Wir beobachteten keine Pseudarthrosen oder Infekte, jedoch Hautirritationen durch das medialseitige Ende des TENS in 4 Fällen. Als seltene Komplikation trat in einem Fall intraoperativ eine Schädigung des Schultereckgelenkes im Sinne einer Tossy-III-Verletzung bei vorbestehender Tossy-II-Verletzung auf. Schlussfolgerung: Die ESIN ist aufgrund der guten funktionellen Ergebnisse eine minimalinvasive Alternative zur Versorgung der dislozierten Klavikulaschaftfrakturen und ermöglicht eine frühe Reintegration in den Alltag.

Abstract

Introduction: The aim of this study was to evaluate the operative technique and the functional and reintegrative results after elastic intramedullary nailing of displaced midclavicular fractures. Materials and Methods: Between 2003 and 2006, 15 patients with midclavicular fractures were treated with elastic nails. Functional results and complications were assessed. Results: The Constant-Score averaged 95,3. Temporary working disability was 22 ± 9 days. Non-union or infections were not observed, but the medial end of the TEN caused skin irritation in 4 cases. A complete acromioclavicular joint separation was observed once after elastic nailing. Conclusion: Elastic intramedullary nailing represents a minimal-invasive alternative for the treatment of displaced midclavicular fractures and allows an early return to daily activities. Functional results are mainly excellent.
