CART-Peptide Immunoreactivity in Enteric Nerves in Patients with Hirschsprung's Disease

Abstract

Aims: Cocaine- and amphetamine-regulated transcript (CART)-peptide is found in the brain and participates in the control of feeding behavior. It is also expressed in the peripheral nervous system and is suggested to have neuromodulatory and/or neurotrophic effects in rat intestine. The aims of this study were to investigate the presence of CART-peptide in the normal ganglionic as well as aganglionic intestine from patients with Hirschsprung's disease and the peptide's possible coexistence with other neurotransmitters. Methods: Intestinal specimens from nine patients with Hirschsprung's disease were examined using immunohistochemistry. A double immunostaining technique was used in order to elucidate the presence of CART-peptide in NOS and VIP-containing enteric neurons. Results: In ganglionic intestine, CART-peptide was found in numerous nerve fibers, predominantly within the smooth muscle layers and in myenteric nerve cell bodies. A high degree of co-localization of CART with NOS and VIP was seen. Only very few CART immunoreactive nerve fibers and no nerve cell bodies were found in the aganglionic intestine. Conclusions: This is the first report on the presence of CART-peptide in the human intestine. In the ganglionic intestine CART was detected mainly in myenteric neurons, while only very few CART‐IR nerve fibers were found in the aganglionic intestine. This, together with the coexistence of CART with NOS and VIP, indicates an intrinsic origin of the CART-containing neurons and suggests that CART may influence NO and VIP-induced effects.

CART-Peptide immuno-réactivité dans l'innervation entérique des patients avec maladie de Hirschsprung

Buts: Cocaïne et amphétamine transcrites peptides (CART) est trouvé dans le cerveau et participe au contrôle du comportement alimentaire. Il est aussi exprimé dans le système nerveux périphérique et il est possible qu'il ait un rôle neuro-modulateur et/ou neuro-trophique pour l'intestin de rat. Le but de cette étude est d'apprécier la présence de CART-peptides dans la cellule ganglionnaire normale, aussi bien que dans l'intestin aganglionnaire des patients avec une maladie de Hirschsprung. Méthodes: Des fragments intestinaux de 9 patients avec une maladie de Hirschsprung étaient examinés en utilisant une immuno-histochimie. Une technique de double coloration était utilisée pour apprécier la présence de CART-peptide. Résultats: Dans l'intestin ganglionnaire, CART6-peptide est trouvé dans de nombreuses fibres nerveux, prédominant dans les plans musculaires lisses et dans les cellules nerveuses myentériques. Une importante colocalisation de CART avec NOS et VIP sont vues. Seulement très peu de fibres nerveuses CART immuno-réactives et aucune cellule nerveuse ne sont retrouvées dans l'intestin aganglionnaire. Conclusion: Ceci est la première publication de la présence de CART-peptide dans l'intestin humain. Dans les cellules ganglionnaires, CART est détectée principalement dans les neurones myentériques alors que très peu de fibres nerveuses CART positives sont retrouvées dans l'intestin aganglionnaire. Ceci, associé avec la co-existence de CART avec NOS et VIP, indique l'origine intrinsèque des neurones contenant CART et suggère que CART ait influencé les effets induits de NOS et de VIP.

Inmunorreactividad a péptido cart en el sistema nervioso entérico de pacientes con enfermedad de Hirschsprung

Objetivos: El péptido transcripto regulado por cocaina y anfetamina (CART) se encuentra en el cerebro y participa en la regulación del comportamiento alimentario. También se expresa en el sistema nervioso periférico y se sugiere que tiene efectos neuromodulatorios y/o neurotróficos en el intestino de rata. Los objetivos de este estudio fueron investigar la presencia de péptido CART en el intestino gangliónico y agangliónico de pacientes con enfermedad de Hirschsprung y su posible coexistencia con otros neurotransmisores. Métodos: Se examinaron inmunohistoquímicamente 9 especímenes de intestino de pacientes con Hirschsprung. Se usó una técnica inmunohistoquímica doble con el fin de elucidar la presencia de péptido CART en neuronas positivas para NOS y VIP. Resultados: En el intestino gangliónico se halló péptido CART en numerosas fibras nerviosas predominantemente enntre las capas musculares y en las neuronas del plexo mientérico. Se observó un alto grado de colocalización de CART con VIP y NOS. En el intestino agangliónico hubo muy pocas fibras y casi ninguna célula positivas. Conclusiones: Esta es la primera vez que se presenta evidencia de la presencia de péptido CART en las neuronas mientéricas del colon gangliónico y de su escasa positividad en el agangliónico. Esto, junto a la coexistencia de CART con VIP y NOS, indica el origen intrínseco de las neuronas que contienen CART y sugiere que CART podría influenciar los efectos dependientes de VIP y NOS.

CART-Peptid-Immunreaktion im enteralen Nervensystem von Patienten mit Morbus Hirschsprung

Zielsetzung: Kokain- und Amphetamin- regulierte Transkriptpeptide (CART) sind im Gehirn nachgewiesen worden. Sie sind an der Kontrolle der Ernährungsmodulation beteiligt. Auch im peripheren Nervensystem werden sie freigesetzt, und es wird vermutet, dass sie im Rattendarm eine neuromodulatorische- und/oder neurotrophe Wirkung haben. Ziel dieser Studie war es, die Anwesenheit von CART-Peptiden im ganglionären, normalen wie auch im aganglionären Darm von Patienten mit Morbus Hirschsprung nachzuweisen und darüber hinaus ihre Koexistenz mit anderen Neurotransmittern zu überprüfen. Methodik: Proben von 9 Patienten mit Morbus Hirschsprung wurden immunhistochemisch untersucht. Dabei wurde eine doppelte Färbetechnik angewandt, um die gleichzeitige Anwesenheit von CART-Peptiden und NOS- sowie VIP-haltigen Neuronen nachzuweisen. Ergebnisse: Im ganglionären Darm waren CART-Peptide in zahlreichen neuronalen Fasern nachweisbar, vor allem zwischen den glatten Muskelschichten und in den Zellkörpern des Plexus myentericus. In hohem Maße wurde eine Ko-Konzentration von CART und NOS- sowie VIP-haltigen Neuronen beobachtet. Im aganglionären Darm hingegen, fanden sich nur wenige CART immunoreaktive Fasern und keine Nervenzellen. Schlussfolgerungen: Dies ist der erste Bericht über die Anwesenheit von CART-Peptiden im menschlichen Darm. Im ganglionären Darm wurden die CART-Peptide vorwiegend in den Neuronen des Plexus myentericus beobachtet, während im aganglionären Darm nur wenige CART-IR-positive Fasern gefunden wurden. Diese Befunde, gemeinsam mit dem gleichzeitigen Vorkommen von NOS und VIP, weisen auf einen intrinsischen Ursprung der CART-haltigen Neurone hin und lassen vermuten, dass CART die Wirkung von NO und VIP beeinflusst.
