Die laparoskopisch assistierte vaginale Hysterektomie - Sinn oder Unsinn?

Zusammenfassung

Die Hysterektomie ist einer der häufigsten operativen Eingriffe in der Gynäkologie. Neben dem klassischen vaginalen und abdominalen Zugangsweg steht zunehmend die Laparoskopie zur Verfügung. Die laparoskopisch assistierte vaginale Hysterektomie ist eine operative Variante, welche die Vorteile der laparoskopischen und vaginalen Präparation bei der Hysterektomie vereinigen kann. Mögliche Vorteile sind die Diagnose und gleichzeitige sichere Sanierung sogenannter Begleitpathologien. Hierzu zählen beispielsweise die Resektion von Endometrioseherden, die ausgedehnte Adhäsiolyse oder die Adnexchirurgie. Auch ist die simultane Adnexektomie unter Sicht zuverlässig möglich. Als nachteilig ist das techniklastige und zeitintensive Operationsprozedere zu werten. In dem folgenden Artikel werden die unterschiedlichen Formen der laparoskopisch assistierten vaginalen Hysterektomie definiert. Das Indikationsspektrum mit Vor- und Nachteilen sowie die technische Durchführung werden dargestellt und aktuell diskutiert.

Abstract

Hysterectomy is one of the most common performed procedures in operative gynecology. Due to this fact several operative strategies developed over the past decades. These strategies include abdominal, vaginal, laparoscopic or laparoscopically assisted vaginal hysterectomy. Laparoscopically assisted vaginal hysterectomy was first introduced in the 1980s proposing a possibly advantageous combination of both, laparoscopic and vaginal surgical preparation. Performing laparoscopically assisted vaginal hysterectomy immediate diagnosis and safe surgical therapy of suspected or unsuspected abdominal pathology is possible. For example, adhesions can be resolved making the later vaginal procedure more secure. Furthermore, the resection of implants of endometriosis is immediately possible and moreover, adnexectomy is an easy and safe procedure laparoscopically. On the other hand, laparoscopically assisted vaginal hysterectomy is a sophisticated, time consuming and therefore expensive operative procedure. In the following article we will define the different forms of laparoscopically assisted or laparoscopic hysterectomies. A description of the surgical procedures will be given. The pros and cons of laparoscopically assisted vaginal hysterectomy will be discussed.
