Differenzialdiagnose inflammatorisches Mammakarzinom: Brustödem nach Thrombose der V. brachiocephalica

Zusammenfassung

Nicht immer ist eine unilaterale Rötung, Volumenzunahme, Überwärmung und Peau d'Orange der weiblichen Brust Ausdruck eines zugrunde liegenden inflammatorischen Mammakarzinoms. Wir stellen drei Fälle mit täuschend ähnlicher Klinik vor, bei denen sich ein Brustödem aufgrund einer vorgängigen venösen Thrombose in der Vena brachiocephalica entwickelte. Zweimal zeigte sich die Klinik bei Leukämiepatientinnen nach Chemotherapie und Knochenmarkstransplantation. Der dritte Fall fand sich bei einer Hämodialysepatientin mit arteriovenöser Fistel. Soweit wir die Literatur überblicken, präsentieren wir hier die ersten Fälle unilateraler Brustödeme mit Thrombosierung der Vena brachiocephalica als Komplikation nach zentraler Venenkathetereinlage bei Leukämiepatientinnen. Ein inflammatorisches Mammakarzinom konnte bei allen drei Patientinnen mittels klinischer Untersuchung, Bildgebung und Haut-/Brustbiopsie ausgeschlossen werden.

Abstract

Three cases of unilateral breast edema due to venous thrombotic occlusion of the brachiocephalic vein mimicking inflammatory breast cancer are presented. Thrombosis as a complication of indwelling central venous catheter placement was associated with arteriovenous fistula in a patient undergoing hemodialysis, and with chemotherapy and bone marrow transplantation in two leukemia patients. To our knowledge, we present the first cases of unilateral breast engorgement in cancer patients due to brachiocephalic venous occlusion complicating the central venous catheter. Inflammatory breast cancer was excluded in all three patients by clinical and imaging work-up as well as by skin/breast biopsy.
