Minimalinvasive Biopsien mammographisch suspekter Läsionen: Indikationen und Durchführung

Zusammenfassung

Nationale und internationale Leitlinien fordern eine präoperative histopathologische Abklärung bei über 90 % der tastbaren und über 70 % der nicht tastbaren malignen Tumoren der Mamma. Eine adäquate minimalinvasive Diagnosesicherung ermöglicht eine interdisziplinäre Therapieplanung mit optimaler Rate von definitiven onkologisch gerechten Operationen in einer Sitzung oder, bei entsprechender Konstellation, die Indikationsstellung zur neoadjuvanten medikamentösen Therapie. Andererseits müssen die interventionellen Eingriffe bei einem breiten Indikationsspektrum entsprechend BI-RADS 4 und 5 mit einer Karzinomwahrscheinlichkeit beginnend bei 2 % Malignome zuverlässig ausschließen. Die Gewebemenge korreliert positiv mit der Diagnosesicherheit und erfordert bei unterschiedlichem histologischen Aufbau ein angepasstes Protokoll hinsichtlich differenter Läsionen wie Herdbefunde, Mikroverkalkungen und Architekturstörungen.

Biopsieverfahren und Protokolle werden systematisch entsprechend Indikation und Durchführung vorgestellt.

Bei der Kommunikation der histopathologischen Ergebnisse von minimalinvasiven Biopsien werden die B-Kategorien verwendet. Fünf B-Kategorien fassen die rein mikroskopischen Ergebnisse zusammen. Sie bilden die Basis für die interdisziplinäre Dignitätseinstufung, die erst nach Überprüfung der radiologisch-pathologischen Korrelation und in der Gesamtzusammenschau der Befunde möglich ist, und den Abklärungsprozess abschließt.

Abstract

National and international guidelines require preoperative histopathological clarification of over 90 % of palpable and over 70 % of nonpalpable malignant tumors of the breast. Adequate minimally invasive diagnosis confirmation allows interdisciplinary therapy planning with an optimal rate of definitive oncologically compliant operations in one session or indication for neoadjuvant medication therapy in the case of a corresponding constellation. Interventions must reliably rule out malignancies for a wide indication spectrum according to BI-RADS 4 and 5 with a carcinoma probability starting at 2 %. The tissue quantity correlates positively to the diagnosis reliability and requires an adjusted protocol with respect to different lesions, such as masses, microcalcifications, and architecture disturbances, in the case of a different structure. Biopsy protocols and procedures are systematically presented according to indication and implementation. B-categories are used to communicate the histopathological results of minimally invasive biopsies. There are five B-categories for purely microscopic results. They form the basis for the interdisciplinary classification of tumor characteristics which is first possible after review of the radiological-pathological correlation and the total synopsis of the findings and concludes the clarification process.
