Kernaussagen
Die flexible Bronchoskopie ist aus der Diagnose und Therapie von Patienten auf der Intensivstation nicht mehr wegzudenken. Die selbständige Anwendung der Methode erfordert spezielle Kenntnisse des Untersuchers über die pathophysiologischen Konsequenzen und Komplikationen bei diesen - meist beatmeten - Patienten. Im Vergleich zur flexiblen Bronchoskopie wird die starre Bronchoskopie nur sehr selten auf Intensivstationen eingesetzt.
Die flexible Bronchoskopie hat trotz technischer Einschränkungen die Diagnostik von Atemwegsinfektionen bei beatmeten und immunsupprimirten Patienten besonders durch den Einsatz indirekter Techniken (wie die bronchoalveoläre Lavage) erweitert. Bei den therapeutischen Indikationen ist sie z. B. zum Freimachen und zur Sicherung der Atemwege und bei der Tracheotomie unverzichtbar geworden. Maßnahmen zur Qualitätssicherung der Methode auch auf Intensivstationen - insbesondere eine strukturierte Aus- und Weiterbildung - sind aus Sicht des Autors aber notwendig.
