Zusammenfassung

Die Inzidenz der periprothetisehen Fraktur steigt mit zunehmender prothetischer Versorgung der Bevölkerung an. Ursächlich können ein Trauma oder pathologische Knochenprozesse sein. Das Patientengut weist in der Regel eine Reihe von Nebenerkrankungen auf, wobei biomechanisch die Osteoporose im Vordergrund steht. Die in der Literatur beschriebene hohe perioperative Komplikationsrate fordert eine individuelle Analyse und genaue Therapieplanung mit dem Ziel einer postoperativen Belastungsstabilität. Die zur Verfügung stehenden Implantate sind vielfältig, jedoch nicht alle für jeden Frakturtyp einsetzbar. Anforderungen an das Implantat sind die Notwendigkeit einer frühfunktionellen Nachbehandlung, ein minimalinvasives Applikationsverfahren sowie ein breites Indikationsspektrum.

The ineidence of periprosthetic fractures increases with rising numbers of Joint replacements. The cause is either trauma or a pathological bone process. Multimorbidity is usually found in this patient population. Biomechanically, the generalized osteoporosis should reeeive attention. The high complication rate in the treatment of periprosthetic fractures emphasizes the importance of an individual and precise preoperative planning leading to füll weight bearing after surgery. There is a wide choiee of implants although not all can be used for each fracture type. The requirements placed on the implant are the necessity of early postoperative rehabilitation, minimal invasive surgery and universal application, independent of prosthetic design and fracture type.
