Multiprofessionelle Kooperation in der Palliativmedizin

Ursprünglich auf den Beginn der Aufklärung mit der französischen Revolution bezogen, würde eine Übertragung dieser Worte auf deutsche palliativmedizinische Versorgungsstrukturen einen großen Fortschritt bedeuten. Die neue Idee beinhaltet eine holistische Betreuungsform von Patienten mit einer begrenzten Lebensprognose - möglichst an dem Ort, an dem der Patient sein Leben beenden will. Die besonderen Zielsetzungen der Palliativmedizin mit ihren multifaktoriellen Problemen, die einen interdisziplinären Ansatz benötigen, um zu multidisziplinären Lösungsansätzen zu gelangen, werden erläutert. Die notwendigen Änderungen in der Zusammenarbeit, die Bedeutung des Palliativteams und weiterer Kooperationen wird dargestellt. Der Kommunikation innerhalb des Teams und mit weiteren Kooperationspartnern kommt in der Palliativarbeit über reine Informationsvermittlung hinaus eine herausragende Rolle zu.

Originally referring to the beginning of the enlightenment (reconnaissance) of the French revolution the transcription of this words regarding to German palliative Care structures would mean a tremendous effort. .The meaning of the new idea is a holistic kind of care for patients with a chronic diesease at the end of their life's, so that they can die as most self determined as possible at a location of their choice. The special aim of palliative care, the need of interdisciplinary cooperation leading to multidisciplinary solutions is pointed out. The meaning of palliative care team as a team with special communication skills in between the team and with further cooperating partners is described. Communication in palliative care means more than telling facts.
