Zusammenfassung
Bei Angst und Panik handelt es sich um ein psychophysisches Gesamtgeschehen, bei dem einmal mehr psychische, einmal mehr körperliche, vegetative Symptomatik im Vordergrund steht. Insbesondere bei Komorbidität mit chronischen Schmerzen sollte bei Schmerzkrisen oder depressiven Dekompensationen an catastrophic reactions gedacht werden. Der menschliche Organismus ist mit der konkreten existenziellen Situation untrennbar verknüpft, seinen Handlungsmöglichkeiten und Selbstaktualisierungen. Bei 10-20 % aller chronischen Schmerzpatienten mit Angstzuständen und schweren Depressionen mit Anpassungsstörungen (vor allem von Frauen) finden sich erhebliche psychophysische Traumatisierungen in der frühen Lebensgeschichte wie sexueller Missbrauch, Psychoterror und Gewalt oder in der späteren wie zum Beispiel Mobbing. Der Zusammenbruch gerade angesichts alltäglicher Belastungen ist im Sinne eines neurobiologischen und neuropsychologischen Geschehens als Katastrophereaktion einzuordnen. Damit ist ein Hinweis auf Verlust der Fähigkeiten zum „Kategorialen” gegeben mit der Folge konkretistischer Fixierung. Die Somatisierung ist ein Phänomen einer solchen existenziellen Einengung auf Konkretistisches. Das hat Konsequenzen für die Therapie, die den Patienten langsam an die Bewältigung erfüllbarer Aufgaben heranführen muss und ihn für komplexere Weltbezüge, wie Arbeitssituation, Konflikte durchzuhalten, neue Beziehungen aufzunehmen, vorzubereiten hat. Wie in der psychotraumatologischen Behandlung sind der Umgang mit einem neurobiologisch zu verstehenden Geschehen, wie zum Beispiel der Intrusion, und das sich zurechtfinden in hermeneutisch und ideografisch zu erschließenden Phänomenen der Existenz mit psychodynamischem Geschehen, untrennbar miteinander verknüpft. In dieser Lernphase können auch auf dem Boden einer tragfähigen Arzt-Patient-Beziehung Psychopharmaka hilfreich sein. Damit erlernt der Patient, sein eigener Arzt zu werden und Dekompensationen, wie der Katastrophenreaktion, vorzubeugen. Die Katastrophenreaktion geht einher mit vegetativer Krise, Affektdurchbrüchen, Panik und Angstüberflutung. - Das Organismische Verstehen bietet die Chance, phänomenologisch vielfältige Krankheitsbilder vom hirnorganisch begründbaren Leiden, über PTBS, Somatisierungsstörungen bis hin zu vordergründig „rein psychischen” eine anthropologische Basis für Handlungssicherheit und grundlegende therapeutische Prinzipien zu geben. Die von Kurt Goldstein in der Behandlung von Hirnverletzten beider Weltkriege in vorbildlicher interdisziplinärer Zusammenarbeit von Neurologen, Biologen und Psychologen erarbeiteten Konzepte zum Organismischen beinhalten nicht nur ein für Klinik und Forschung wegweisendes erweitertes Verständnis von Biologie, sondern auch kritisches Potenzial für eine auf psychogene Konzepte eingeengte Psychosomatik, mit wichtigen Perspektiven für die Beziehung zur Psychiatrie.
Abstract
Anxiety and panic states are a pschophysical occurrence by which sometimes more psychical and other times more physical vegetative symptoms predominate. In particular, in cases of comorbidity with chronic pain, pain crises or depressive decompensations should be considered as catastrophic reactions. The human organism is inseparably linked with the concrete, existential situation, its possibilities of action and self-actualisation. In 10-20 % of all chronic pain patients suffering from states of anxiety and severe depression with adjustment disorders (above all women), one will find a considerable psychopsychical traumatisation in the earlier life history, such as sexual abuse, psychoterror and violence or in the more recent life history, e. g., mobbing. A breakdown just in the face of daily stress should be considered as a catastrophic reaction in the sense of a neurobiolgical and neurophyschological event. Thus, this is the suggestion for a loss of the categorial ability with the result of a concerete fixation. Somatisation is the phenomenon of such an existential restriction to concretisation. This has the consequences for therapy that the patient must slowly be led up to the accomplishment of the feasible tasks and be prepared to take on more complex affairs suich as working situations, to withstand conflicts and to enter into new relationships. As in psychotraumatological treatment, dealing with an occurrence that can be neurobiologically understood, such as for example, intrusion, and finding one's way in the phenomena of existence with psychodyamic events that must be encountered hermeneutically and ideographically are inseparably linked. In this learning phase psychopharmaceutics may be of help on the basis of a solid doctor-patient relationship. In this way the patient learns to be his/her own doctor and to guard against decompensations like the catastrophic reaction. The catastrophic reaction is accompanied by vegetative crises, affect breakthroughs, panic and anxiety floods. - Organismic understanding offers the chance to provide an anthropological basis for security of action and fundamental therapeutic principles for phenomenologically diverse clinical entities to - in principle - “purely psychical” problems. The concept of organismics developed by Kurt Goldstein in exemplary interdisciplinary cooperation between neurologists, biologists and psychologists for the treatment of brain-damaged patients in both World Wars encompasses not only a pioneering and expanded understanding of biology for clinic and research but also a critical potential for psychosomatics restricted to psychogenic concepts with important perspectives for the relationship with psychiatry.
