Zusammenfassung
Hintergrund Bislang waren Versuche, einzelne Biomarker zur Diagnostik eines Schlaganfalls zu nutzen, ohne klinisch relevanten Erfolg. Ein kommerziell erhältliches Testsystem (Triage® Stroke-Panel) misst vier Biomarker und ermittelt daraus einen dimensionslosen Multimarker-Index™ (MMX). Ziel der prospektiven Studie war es zu untersuchen, ob bei Patienten mit nachgewiesenem Schlaganfall die MMX-Werte gegenüber Patienten mit anderen neurologischen Erkrankungen erhöht sind. Methoden Es wurden Blutproben von 70 Patienten mit der klinischen Symptomatik eines in den letzten 24 Stunden akut aufgetretenen Schlaganfalls und Nachweis einer entsprechenden Läsion in der zerebralen Bildgebung untersucht. Als Kontrolle dienten 70 akut neurologisch erkrankte Patienten ohne klinischen und bildmorphologischen Nachweis eines Schlaganfalls. Ergebnisse In der Schlaganfallgruppe wurde ein signifikant höherer MMX-Wert (Mittelwert 4,6; Standardabweichung 1,73) als in der Kontrollgruppe (Mittelwert 2,9; Standardabweichung 1,89) gemessen (p < 0,001). Nach Korrektur für Alter und Geschlecht fand sich in der Schlaganfallgruppe ein durchschnittlicher MMX-Wert von 4,3 verglichen mit 3,0 in der Kontrollgruppe (p < 0,05). Ein MMX-Wert von ≥ 4,0 wurde bei 53/70 der Schlaganfallpatienten sowie bei 23/70 der Kontrollpatienten ermittelt. Für diesen Grenzwert ergeben sich eine Sensitivität von 76 % und eine Spezifität von 67 % für die Schlaganfalldiagnose. Fazit Der MMX-Wert ist bei Patienten mit nachgewiesenem Schlaganfall gegenüber anderen akut neurologisch erkrankten Patienten erhöht. Die ermittelte Sensitivität und Spezifität erlauben jedoch nicht eine hinreichend sichere Diskriminierung zwischen Schlaganfallpatienten und Kontrollen. Die Anwendung des untersuchten Testsystems in der Akutdiagnostik eines Schlaganfalls erbringt zum jetzigen Zeitpunkt keinen eindeutigen Zusatznutzen.
Abstract
Background and purpose Until now, the test of single biomarkers in peripheral blood for the diagnosis of a stroke has not been successful in clinical terms. A commercially available test system (Triage® Stroke-Panel) measures four biomarkers to calculate a dimensionless Multimarker Index™ (MMX). The aim of this prospective study was to investigate whether the MMX was elevated in stroke patients compared with other neurological patients. Methods Blood samples were investigated from 70 patients with clinical symptoms of an acute stroke within the previous 24 hours and in whom a stroke could be demonstrated by a brain scan. As a control, 70 acutely ill neurological patients were chosen who exhibited neither clinical symptoms nor imaging evidence of a stroke. Results The stroke group had a significantly higher MMX value (mean 4.6; standard deviation 1.73) than the control group (mean 2.9; standard deviation 1.89) (p < 0.001). Following correction for age and sex, an average MMX value of 4.3 was obtained in the case of the stroke group compared with 3.0 for the control group (p < 0.05). An MMX value of ≥ 4.0 was observed in 53/70 of the stroke patients and 23/70 of the controls, affording a sensitivity of 0.757 and a specificity of 0.671 for stroke diagnosis. Conclusions The MMX value is elevated in patients with confirmed stroke compared to other acutely ill neurological patients. However, there is a substantial overlap of the two groups which makes it impossible to distinguish between the two groups clearly. At present, the use of the investigated test system does not seem to be of additional value for the acute diagnosis of stroke.
