Die geplante Pflege, abgebildet im Pflegeprozess, wird seit etwa 20 Jahren in der deutschen Pflege diskutiert und sollte ihre Anwendung - da gesetzlich vorgeschrieben - auch in der nephrologischen Pflege finden. Eine fixe Definition der Begriffe Pflegeprozess und Pflegeplanung gibt es nicht. Prinzipiell verstehen wir unter dem Begriff Pflegeprozess eine Methode der Pflege, die sowohl Problemlösende wie auch Beziehungsaspekte einbezieht. Planung meint den schriftlichen Teil, in dem der Prozess festgehalten wird. Der Pflegeprozess besteht aus mehreren Phasen. Vor der Diagnose, der Klärung der Ziele und der Festlegung von Interventionen steht eine ausführliche Pflegeanamnese und ein nephrologisches Assessment, das ein interdisziplinäres Team einschließen sollte. Voraussetzung für den Pflegeprozess ist das dezentral angelegte System der Bezugspflege, in dem eine Pflegekraft eine überschaubare Zahl von Patienten betreut.
Planned patient care within the general health care process has been under discussion in Germany for about twenty years and should also be applied to nephrology patients considering the fact that patient care is legally prescribed. However, the concepts of how to proceed and how to plan patient care have not been precisely defined. Basically, the „process of patient care” should include aspects of solving problems as well as aspects of patient/care personnel relations. „Planning” signifies writing down how to proceed. Care procedure has several phases and steps. First of all, there must be a detailed anamnesis of care and a nephrological assessment which should include an interdisciplinary team , followed by diagnosis, definition of aims and of interventions. Success of the care process depends on the decentralised system of nurse/patient relation care where each female or male nurse cares for a manageable number of patients.
