Zusammenfassung:
Die chronische Sprunggelenksinstabilität ist ein Zustand, der charakterisiert ist durch häufige Sprunggelenksdistorsionen, Schwierigkeiten beim Gehen und Rennen auf unebenem Gelände und häufig mit Schmerzen verbunden. Gute Sprunggelenksdistorsionen führen in 20-30 % zur chronischen Sprunggelenksinstabilität. Abgestützt auf wissenschaftliche Untersuchungen wird neben den bisherigen rekonstruktiven Operationsmöglichkeiten zur Behandlung der chronischen Sprunggelenksinstabilität als auch der akuten Verletzung mehr und mehr dem propriozeptiven Training Beachtung geschenkt. Die vorliegende Untersuchung zeigt, daß die Reaktionszeit nach einer raschen Winkeländerung im Sprunggelenk, welche durch einen Falltüren-Mechanismus ausgelöst wird, bei Patienten mit einer chronischen Sprunggelenksinstabilität im Vergleich zu Kontrollpersonen um etwa 15 ms verlängert ist. Wir schließen daraus, daß die verzögerte propriozeptive Reaktionsfähigkeit auf rasche Winkeländerungen im Sprunggelenk eine der möglichen Ursachen der chronischen Sprunggelenksinstabilität darstellen könnte und daß somit das propriozeptive Training in die Behandlung von akuten und chronischen Sprunggelenksinstabilitäten integriert werden sollte.
Abstract
Chronic lateral instability of the ankle, a condition with frequent ankle sprain episodes, difficulties to walk and run on uneven ground and often pain, occurs in 20-30 % after acute ankle sprains. The treatment of CLI as well as of acute ankle sprains have more and more in addition to operative reconstructions been focused on proprioceptive training, which is in concordance with the results of recent research results. The present study shows that the reaction time after sudden angular displacement of the ankle on a trapdoor is increased by approximately 15 ms in patients with chronic lateral instability compared to control persons. We conclude that delayed proprioceptive response to sudden angular displacement of the ankle can be one of the causes to chronic lateral instability of the ankle and that proprioceptive training should be included in the treatment of acute and chronic ankle disabilities.
