Definition und Pathophysiologie

• Der Begriff „funktionelle Bauchschmerzen” beschreibt sehr häufig wiederkehrende oder (nahezu) kontinuierliche abdominale Schmerzen, die dem Verdauungstrakt zugeschrieben werden, aber nicht oder nur unwesentlich mit gastrointestinalen Funktionen oder deren Störungen assoziiert sind.

• Es gibt zahlreiche Hinweise darauf, dass Veränderungen des endogenen schmerzmodulierenden Systems oder periphere neuropathische Schmerzmechanismen eine wichtige pathophysiologische Rolle spielen.

• Darüber hinaus besteht eine signifikante Assoziation chronischer abdominaler Schmerzen mit affektiven Erkrankungen, insbesondere Angststörungen und Depressionen.

Diagnostik

• Die Diagnosestellung basiert auf dem Vorliegen definierter Symptomkriterien sowie einer unauffälligen Ausschlussdiagnostik, deren Umfang im Spannungsverhältnis zwischen diagnostischer Sicherheit einerseits und zunehmender Belastung des Patienten sowie des Gesundheitssystems andererseits steht.

Therapie

• Grundlage einer aussichtsreichen Behandlung funktioneller Bauschmerzen ist ein gutes Arzt-Patienten-Verhältnis.

• Zur erfolgreichen Therapie sind oft multidimensionale und interdisziplinäre Therapiekonzepte erforderlich.

• Es stehen hierfür sowohl empirische medikamentöse Optionen, unter anderem ein Einsatz von zentralnervös wirkenden Substanzen wie Antidepressiva und Antikonvulsiva, als auch psychotherapeutische Behandlungsansätze zur Verfügung.
