Zentralvenöse Katheter: Diagnostik von Komplikationen und therapeutische Optionen

Zusammenfassung

Für intensivierte Therapien haben zentralvenöse Katheter zur Diagnostik und Therapie in der modernen Medizin einen hohen Stellenwert erlangt. Der Radiologe kann bei der Implantation, Diagnostik von Komplikationen und der Therapie von Katheterkomplikationen eine wichtige Rolle spielen. Der vorliegende Artikel gibt einen Überblick über die verschiedenen zentralvenösen Kathetertypen, deren Indikationen sowie Vor- und Nachteile.

Fehllagen von Kathetern sind häufig schon auf dem konventionellen Röntgenbild zu erkennen. Mit interventionell-radiologischen Methoden ist oft eine Katheterkorrektur möglich. Darüber hinaus werden therapeutische Optionen zur Behebung thrombotischer Komplikationen (venöse Thrombose, Katheterokklusion, Fibrinscheide) vorgestellt und diskutiert. Bei infektiösen Komplikationen ist oft eine Katheterentfernung und ggf. Neuimplantation angezeigt.

Abstract

For modern medicine central venous catheters play an important role for diagnostic and therapeutic options. Catheter implantation, complication detection and therapy of catheter complications are an increasing demand for the radiologist. The review article provides an overview of different catheter types, their indications, advantages and disadvantages.

Catheter malpositions are usually detectable in conventional X-ray. Most malpositions are correctable using interventional-radiological techniques. In addition therapeutical options for thrombotic complications (venous thrombosis, catheter occlusion, fibrin sheath) are discussed. In case of an infectious catheter complication, usually a catheter extraction and re-implantation is nessessary.

KathetertypenBei ZVK werden nicht getunnelte (Shaldon, PICC, Swan-Ganz), getunnelte (Hickman, Broviac, Demers) und komplett implantierte Katheter (Portkatheter) unterschieden.

Katheterlage

• Über die optimale Katheterlage im zentralvenösen System besteht keine Einigkeit, die Katheterspitze sollte jedoch weder in einer kleinlumigen Vene noch zu tief atrial liegen.

• Meist wird empfohlen, dass sich die Katheterspitze im konventionellen Röntgenbild 4 cm tiefer als die Karina projizieren sollte; bei Dialysekathetern auf Höhe des kranialen rechten Vorhofs.

• Varianten im venösen System (insbesondere persistierende linke und gedoppelte V. cava superior) können zu einem atypischen Katheterverlauf führen, der eine primär falsche Position des Katheters suggeriert.

Katheterkomplikationen

• Es werden periinterventionelle (insbesondere Blutung, Herzrhythmusstörungen), Früh- (insbesondere Kathetermigration und -okklusion) und Spätkomplikationen (insbesondere Infektion und venöse Thrombose) unterschieden.

• Katheterfehllagen werden analog ihres zeitlichen Auftretens in primär oder sekundär als auch anhand der Lokalisation der Fehllage in arteriell, venös oder extravaskulär eingeteilt.

• Das Pinch-off-Syndrom bezeichnet die extravasale Kompression des Katheters zwischen der Klavikula und der ersten Rippe bei einer medialen Punktion der V. subclavia.

• Katheterfrakturen werden durch Mobilität des Katheters, Katheterkompressionen oder Knickbildungen begünstigt.

• Zu den katheterassoziierten Thrombosen zählen Thrombosen der kathetertragenden Vene, wandständige, murale Thromben, Thrombosen der Katheterlumina oder Thrombin-/Fibrinüberzüge der Katheter („Fibrin Sheath”).

• Endothelschäden während der Implantation, Schäden durch Bewegung des Katheters und der Katheterspitze, Flussveränderungen durch den Katheter und inflammatorische Reaktionen der Wand auf den Fremdkörper können letztlich zu Stenosen der kathetertragenden Vene führen.

• Infektionen sind als Katheterkolonisation, Infektion an der Hautaustrittsstelle, Tunnelinfektion, Porttascheninfektion und als katheterassoziierte Sepsis möglich.

• Der Radiologe kann sowohl bei der Diagnose einer Katheterkomplikation als auch bei der interventionellen Therapie der Komplikationen einen wichtigen Beitrag leisten.
