Hörverlust durch Hochdosis-Carboplatintherapie

Zusammenfassung

Hintergrund: Carboplatin wird als ein nicht oder nur wenig ototoxisches Chemotherapeutikum angesehen. Methode: Wir berichten über 3 Patienten mit Rezidiv eines Hodenkarzinoms nach Cisplatintherapie, die durch eine Hochdosis-Carboplatintherapie einen Hörverlust erlitten. Ergebnisse: Tonschwellenaudiometrisch bestand ein Carboplatin-induzierter Hörverlust vorwiegend im Mittel- und Hochfrequenzbereich bis 45 dB bei 3 kHz und 55 dB bei 8 kHz. Die transitorisch evozierten otoakustischen Emissionen ließen sich bei 2 der 3 Patienten nach Carboplatintherapie nicht mehr nachweisen. Schlußfolgerung: Nach vorangegangener Chemotherapie mit Cisplatin kann auch Carboplatin in hoher Dosierung einen Hörverlust in den mittleren und hohen Frequenzen erzeugen.

Summary

Background: Carboplatin is regarded as a non-ototoxic or low-grade ototoxic chemotherapeutic agent. Method: We report on three patients with a recurrence of testicular cancer after cisplatin chemotherapy who suffered hearing loss after subsequent highdose carboplatin therapy. Results: Audiometry demonstrated carboplatin-induced hearing loss primarily in the midrange and high frequencies up to 45 dB at 3 kHz and up to 55 dB at 8 kHz. In two of three patients, transitory-evoked otoacoustic emissions were absent after carboplatin therapy. Conclusion: Following first-line cisplatin chemotherapy, salvage treatment with high-dose carboplatin can generate hearing loss in the middle and high frequencies.
