Zusammenfassung
Die prophylaktische systemische Verabreichung von Antibiotika zur Vermeidung postoperativer Wundinfektionen wird für größere Eingriffe im allgemeinen empfohlen. Der Operateur muß dabei das für den Patienten und den geplanten Eingriff optimale Antibiotikum in ausreichender Dosierung und Dauer applizieren, um das Auftreten von Wundkomplikationen zu minimieren. Andererseits muß er kostengünstig handeln und darüber hinaus nach Wegen suchen, das Auftreten bakterieller Resistenzen zu vermeiden. Somit dürfen prophylaktische Antibiotika kein zu breites Spektrum aufweisen und nur kurzfristig verabreicht werden.
Um einer Optimierung der Antibiotikaprophylaxe in der Kopf-Hals-Chirurgie näherzukommen, wurde in einer prospektiven randomisierten Studie die Effizienz einer perioperativen Ein-Dosis-Prophylaxe mit Cefuroxim (Zinacef®) bei Patienten geprüft, die sich einer lateralen Parotidektomie, einer Neck dissection (ohne transkutane Eröffnung des Pharynx) oder einer endonasalen Nebenhöhlenoperation unterzogen. Die Ein-Dosis-Prophylaxe wurde dabei der 24-Stunden-Prophylaxe mit dem selben Antibiotikum gegenübergestellt. Hierbei zeigten beide Prophylaxeschemata für die geprüften Eingriffe gleichmäßig gute Resultate. Bei 106 Patienten trat postoperativ keine Wundinfektion auf. Die perioperativen Konzentrationen von Cefuroxim in Serum und Gewebe zeigten in der weit überwiegenden Mehrheit der Fälle suffiziente Werte gegenüber den für Wundinfektionen typischen Keimen.
Die perioperative Ein-Dosis-Prophylaxe mit Cefuroxim erweist sich somit als geeignet zur Vermeidung postoperativer Wundinfektionen in der Kopf-Hals-Chirurgie.
Summary
Perioperative systemic antibiotic cover is usually recommended in major head and neck surgery with the aim of preventing postoperative wound infection. The surgeon must administer the drug most suitable for the particular Operation and patient at an adequate dosage and over an adequate period. On the other hand, he has an Obligation to minimize costs and find ways of avoiding the development of bacterial resistance to the antibiotic. This means that prophylactic anti-biotics may not have too broad an antibacterial spectrum and may only be used over the short term.
In an attempt to improve antibiotic prophylactic in head and neck surgery, we tested the effectiveness of a perioperative single-dose antibiotic cover with Cefuroxim (Zinacef™) in patients undergoing parotidectomy, sinus surgery or neck dissection with no transcutaneous exploration of the pharynx. This single-shot Prophylaxis was compared with a three-shot, 24-hour regimen using the same antibiotic. The two regimens were equally effective in preventing wound infection and no case of infection was observed among the 106 patients involved. Preoperative concentrations of Cefuroxim were measured in serum and tissue, and the large majority of specimens showed effective concentrations against the typical wound infection bacteria.
In conclusion, a perioperative single-dose administration of Cefuroxim proved to be suitable Prophylaxis against postoperative wound infection in head and neck surgery.
