Zusammenfassung
Für die Behandlung chronischer pharmakoresistenter Epilepsien gewinnt die Resektion epileptogener Gehirnareale zunehmend an Bedeutung. Bei einer Auswertung von 279 epilepsiechirurgischen Resektaten des Epilepsiezentrums der Universität Bonn fanden sich in 89 % aller vollständig beurteilbaren Praparate spezifische histopathologische Veränderungen. Tumoren niedrigen Malignitätsgrads wie Gangliogliome, pilozytische Astrozytome und dysembryoplastische neuroepitheliale Tumoren waren unter den fokalen Läsionen am häufigsten vertreten. Glioneuronale und vaskuläre Fehibildungen und die Ammonshornsklerose wurden ebenfalls oft beobachtet. Die Ergebnisse deuten darauf hin, dass chronischen pharmakoresistenten Epilepsien häufig strukturelle Veranderungen änderungen zugrunde liegen.
Summary
The surgical treatment of chronic epilepsies is increasing rapidly. A review of 279 surgical specimens of patients with chronic pharmacoresistant epilepsies from the University of Bonn Medical Centre revealed specific histopathological findings in 89 % of all well-preserved specimens. The most frequent focal lesions were low-grade brain tumours such as gangliogliomas, pilocytic astrocytomas and dysembryoplastic neuroepithelial tumours. Other common findings were glioneuronal and vascular malformations and Ammon's horn sclerosis. This suggests that chronic intractable epilepsies are frequently caused by structural alterations.
