Zusammenfassung
Innerhalb der letzten 5 Jahre wurden bei 10 Patienten der Universitäts-Augenklinik Freiburg Lid- und Orbitatumoren exzidiert, die 4 Wochen bis 2 Jahre nach Nasennebenhöhleneingriffen mit anschließender Blutung in die vordere Orbita und die Lidweichteile aufgetreten waren. Bei der histologischen Untersuchung fanden sich Fremdkörpergranulome um Lipidvakuolen unterschiedlicher Größe mit zahlreichen Riesenzellen. Das mikroskopische Bild war typisch für sogenannte Paraffingranulome. In drei Fällen konnten durch eine NMR-spektrografische Untersuchung im Granulationsgewebe hohe Konzentrationen von n-Alkanen nachgewiesen werden, die Hauptbestandteile von Paraffinöl und Nebenbestandteile von Vaseline sind. Die Fremdkörpergranulome wurden wahrscheinlich durch die üblicherweise am Ende von Nasennebenhöhleneingriffen durchgeführte Tamponade der Wundhöhle mit paraffinhaltigen Salben verursacht, wobei Salbenbestandteile durch die Blutung ins Gewebe eingeschwemmt wurden.
Summary
During the last five years 10 patients received surgical treatment in our eye clinic for tumours of the eyelids and the orbit that had developed after endonasal sinus surgery. In most patients the first signs of lid or orbital involvement had occurred 4 weeks after ENT surgery and after a unilateral or bilateral postoperative haematoma had subsided. The histological examination of the surgical specimens showed chronic sclerosing lipogranuloma containing large vacuoles surrounded by epitheloid and giant cells, typical of a so-called paraffin granuloma. MR-spectroscopy detected high concentrations of paraffin in the specimens of 3 patients. Since paraffin is a constituent of many ointments, we assume that the granulomatous reaction was elicited by the ointment tampons routinely applied to the wound area following sinus surgery. The bleeding acted as a vehicle to distribute the paraffin into the surrounding tissues.
