Zusammenfassung
An 52 Leichen-Schädelhälften wurde zuerst eine endoskopische Sphenoethmoidektomie durchgeführt. Anschließend erfolgte die anatomische Präparation mit Erfassung einer ganzen Reihe von Meßdaten wie Vorwölbung des Canalis opticus, knöcherne Wandstärke über dem N. opticus und der Arteria carotis interna u. a., und die Anfertigung histologischer Schnitte in ausgewählten Fällen. Durch die Anwendung anatomischer, histologischer und endoskopischer Untersuchungsmethoden jeweils an ein- und demselben Präparat lassen sich zwei Gefahrenpunkte an der hinteren Rhinobasis klar darstellen: Die unmittelbare topografische Beziehung des Canalis opticus und der Arteria carotis interna zur lateralen Wand der Keilbeinhöhle beziehungsweise der hinteren Siebbeinzellen. Onodi-Zellen wurden in 42% gefunden! Die knöcherne Wandstärke an der Stelle der stärksten Vorwölbung des Canalis opticus betrug im Durchschnitt 0,28 mm, wobei knöcherne Dehiszenzen in 12% der Fälle vorhanden waren. Die zugrundeliegenden anatomisch-morphologischen und mikroanatomischen Daten sowie die klinisch-chirurgische Bedeutung der Befunde werden dargestellt.
Summary
In 52 cadaveric half-heads and endoscopic sphenoethmoidectomy was performed. Subsequently an anatomical preparation with registration of specific data like bulging of the optic canal, thickness of the bony wall covering the optic nerve and the internal carotid artery was achieved, followed by histological sections in specially selected cases. Thus the two key areas of major surgical hazard in the posterior rhinobasis could be clearly demonstrated: the immediate topographic relation of the optic canal and the internal carotid artery to the lateral wall of the sphenoid sinus and the cells of the posterior ethmoid, respectively. Onodi cells of varying degrees were found in 42% of all cases. The thickness of bony wall over the maximum bulging of the optic canal averaged 0,28 mm. Bony dehiscences could be demonstrated in 12% of the cases. The technique of data acquisition, the anatomical and histological findings as well as their clinical and surgical relevance are discussed.
