Zusammenfassung
An auf 37 °C temperierten menschlichen Felsenbeinpräparaten wurden Temperaturmessungen nach Spülungen mit Luft bzw. Wasser durchgeführt. Außerdem wurde die Stärke der Nystagmusreaktionen bei gesunden Versuchspersonen nach Spülungen mit den beiden Reizmedien verglichen.
Die Nystagmusreaktionen sind sowohl bei der Warmwie bei der Kaltreizung nach Spülung mit Luft signifikant geringer als nach Spülung mit Wasser. Dies wird bestätigt durch die Ergebnisse der Temperaturmessungen. Sie zeigen dass die maximale Temperaturänderung im Gehörgang zwar unabhängig vom Reizmedium ist, die maximale Temperaturänderung im lateralen Bereich des horizontalen Bogengangs jedoch ist nach Spülungen mit Luft erheblich geringer als nach Spülungen mit Wasser. Der Grund für die schwächere Reizung nach Luftspülung ist die erheblich geringere Wärmekapazität und schlechtere Wärmeleitzahl von Luft gegenüber Wasser.
Messungen an Präparaten mit perforierten Trommelfellen zeigen, dass die Verwendung eines Thermostimulators nach Scherer, einer von Wasser durchströmten, den Gehörgang auskleidenden Silikonblase, den am ehesten reproduzierbaren thermischen Reiz ergibt. Spülungen mit Luft führen infolge der Verdunstung von Feuchtigkeit in der Paukenhöhle auch bei Warmreizung zu einer Abkühlung im horizontalen Bogengang. Mit Hilfe eines Sondenmikrophons durchgeführte Messungen im Gehörgang während einer Luftspülung ergaben Schallpegel von bis zu 125 dBA.
Diese Ergebnisse zeigen, dass im Interesse deutlicher, reproduzierbarer und gut auswertbarer Nystagmusreaktionen sowie wegen der geringeren Lärmbelastung des Patienten Wasser als Reizmedium vorzuziehen ist.
Summary
The influence of the Stimulus medium in thermal vestibular tests has been investigated by many authors, comparing nystagmus reactions after air and water irrigations. We first measured temperature changes in the ear canal and in the horizontal semicircular canal after both kinds of stimuli, using human temporal bone preparations, which were placed in a water bath of 37 °C. Irrigations were done with air of 45 and 27 °C with a flow of 5 l/min during 45 s and with 100 ml water of 44 and 30 °C during 30 s.
The maximum temperature changes in the ear canal are almost equal after both kinds of irrigation (see Figs. 2 and 3). The change of temperature, however, lasts longer after water irrigation. The time constants are 20 and 57 s for the decay after warm and cold water irrigation and 9 and 20 s after air irrigation, respectively. The maximum temperature change in the lateral part of the horizontal semicircular canal is 0.4 and - 0.3° after warm and cold irrigation with water. The corresponding values after irrigation with air are 0.1 and - 0.15°.
In addition, we investigated the effect of irrigating temporal bone preparations with perforated tympanic membranes (Fig. 4). After irrigation with air of 45 °C, the temperature in the horizontal semicircular canal decreased due to evaporation of humidity in the tympanic cavity. The equipment of choice for the irrigation of such ears is a stimulator reported by Scherer, where water flows through a silicon bubble which coats the ear canal. Using this stimulator the maximum temperature changes werde 0.2 ° as well in preparations with intact as with perforated tympanic membranes.
Tests on 10 healthy persons, comparing nystagmus responses, showed that the maximum values of slow phase velocity and number of beats are significantly lower for air than for water irrigation. This is in good agreement with the results of temperature measurements in the horizontal semicircular canal. In our opinion, the reason for this difference is the one hundred times smaller amount of heat and the twentyfive times smaller heat conductivity coefficient of air compared to water.
Sound level measurements in the ear canal during air irrigations showed values up to 125 dBA.
With regard to distinct, reproducible and easy to evaluate nystagmus reactions and to patient comfort, water is the preferable medium for stimulation in thermal vestibular tests.
