Zusammenfassung
Die Infiltrationsanästhesie besitzt nach wie vor einen hohen Stellenwert bei der operativen Behandlung von Patienten in der Hals-Nasen-Ohren-Heilkunde. Die Injektion von Lokalanästhetika in zum Teil sehr stark durchblutete Gebiete bedingt stets die Gefahr hoher Plasmakonzentrationen des Lokalanästhetikums, verbunden mit dem Auftreten unerwünschter Nebenwirkungen, wie Fallberichte wiederholt belegen. Mit der vorliegenden Untersuchung werden Plasmakonzentrationsverläufe demonstriert, wie sie bei Routineeingriffen zu Tonsillektomien und Tympanoplastiken auftreten.
Material und Methodik : Bei insgesamt 47 Patienten wurden die Plasmakonzentrationsverläufe unmittelbar nach Abschluß der Injektion in kurzen Intervallen bis zu einer Stunde im zentralen Venenblut gemessen. Gruppe 1: Lidocain 0,5% mit Adrenalin (1:200 000) 15-25 ml zur Tonsillektomie (n = 18). Gruppe 2: Lidocain 0,5% mit Adrenalin (1:200 000) 8-15 ml zur Tympanoplastik (n = 15). Gruppe 3: Prilocain 1% mit Adrenalin (1:200 000) 8-15 ml zur Tympanoplastik (n = 15). Die Infiltrationsanästhesien bei den Patienten der Gruppe 2 wurden aus operationstaktischen Gründen zusätzlich zur Allgemeinanästhesie durch den HNO-Arzt appliziert, während die Patienten der Gruppen 1 und 3 ausschließlich unter Infiltrationsanästhesie operiert wurden.
Ergebnisse : Innerhalb der ersten Minuten nach Abschluß der Injektionen traten in allen drei Gruppen zum Teil erhebliche Plasmakonzentrationen auf, die im Bereich des Schwellenwertes zur Auslösung unerwünschter systemischer Wirkungen lagen. Vor allem bei den Patienten der Gruppe 1 (Tonsillektomien) traten vereinzelt Plasmakonzentrationen von 4-7 µg/ml innerhalb der ersten Minute auf. Die höchsten Mittelwerte traten jeweils innerhalb der ersten fünf Minuten auf und betrugen für Gruppe 1 2,07 µg/ml, für Gruppe 2 0,45 µg/ml und für Gruppe 3 1,15 µg/ml.
Diskussion : Bei Infiltrationsanästhesien in den gut vaskularisierten Operationsgebieten der HNO-Heilkunde muß, trotz der Anwendung der als sicher geltenden Gesamtdosen der Lokalanästhetika, vor allem in der Frühphase mit zum Teil sehr hohen Plasmakonzentrationen der betroffenen Substanzen gerechnet werden. Trotz sorgfältiger Injektionstechniken (wiederholter Aspirationstest in zwei Ebenen) scheinen zumindest partielle intravasale Injektionen nicht immer vermeidbar zu sein, wie die pharmakokinetische Interpretation individueller Kurvenverläufe ergibt. Sinnvoll erscheint es uns daher, auch bei diesen Patienten, neben einem sicheren venösen Zugang und prophylaktischer Sauerstoffgabe, eine Überwachung (EKG, RR, HF) zu fordern, bei der neben der sofortigen Erkennung bedrohlicher Störungen auch deren unverzügliche Behandlung gewährleistet ist; die stand-by-Funktion des Anästhesisten halten wir daher für sicherer und somit vorteilhaft.
Summary
Infiltration anaesthesia is still relevant for the surgical treatment of patients in otorhinolaryngology. The injection of local anaesthetics in well vascularised areas constantly causes the danger of high plasma concentrations of local anaesthetics combined with undesirable side effects. In our study we tried to determine the development of plasma concentrations of local anaesthetics in patients scheduled for routine tonsillectomies and tympanoplasty.
Materials and Methods : In 45 patients the development of plasma concentrations was measured immediately after the injection at short intervals; the samples were obtained between 1 minute and 60 minutes after the first injection. Group 1: Lidocaine 0,5% with epinephrine (1:200 000) 15-20 ml for tonsillectomy (n - 18). Group 2: Lidocaine 0,5% with epinephrine (1:200 000) 8-15 ml for tympanoplasty (n = 15). Group 3: Prilocaine 1% with epinephrine (1:200 000) 8-15 ml for tympanoplasty (n = 15). For tactical reasons infiltration anaesthesia for the patients of group 2 was - in addition to general anaesthesia - applied by the otorhinolaryngologist, whereas the patients of groups 1 and 3 were operated exclusively under local anaesthesia.
Results : Within the first minute after the initial injection plasma concentrations of the local anaestethic increased close to toxic threshold levels that are associated with undesirable systemic side effects. In the patients of group 1, who underwent tonsillectomy, plasma concentrations of 4-7 µg/ml were found during the first minute. The highest average values always appeared within the first five minutes: group 1 2,07 µg/ml, group 2: 0,45 µg/ml, and group 3: 1,15 µg/ml.
Discussion : With infiltration anaesthesia in well vascularised areas it may happen that there are - mainly in the early stage - high plasma concentrations of the applied substances, although the total dose was below the known maximum. Despite careful technique (repeated aspiration test in two levels) at least partial intravascular injections are apparently not always avoidable according to the pharmakocinetic data. Our results demonstrate that in addition to a safe peripheral venous line and prophylactic oxygen therapy, intraoperative monitoring of blood pressure, heart rate, electrocardiogram and verbal patient monitoring is of advantage in this group of patients. In our opinion the “standby function” of an anaesthesiologist can avoid severe complications.
