Zusammenfassung
Als gravierende Nebenwirkung der Immunsuppression bei Organtransplantationen ist die Inzidenz maligner Tumoren gegenüber der altersentsprechenden Normalbevölkerung signifikant um das Dreifache erhöht. Unter Behandlung mit den konventionellen Immunsuppressiva (Azathioprin, Steroide und Anti-lymphozytenglobulin) treten dabei vor allem Karzinome der äußeren Haut (40%) auf, während bei dem routinemäßig in die klinische Anwendung eingeführten neuen Immunsuppressivum Cyclosporin A ein vergleichsweises Auftreten von malignen Non-Hodgkin Lymphomen (41%) zu beobachten ist und die Patienten eher durch potentiell lebensbedrohende Tumoren innerer Organe gefährdet sind. In der vorliegenden Arbeit wird ein Fall eines Larynxkarzinomes diskutiert, das während immunsuppressiver Behandlung mit Cyclosporin A 10 Monate nach Herztransplantation auftrat.
Summary
As a serious side effect of immunosuppressive therapy following organ transplantation, the incidence of malignancies shows a significant threefold increase compared with the agematched general population. Under treatment with conventional immunosuppressive drugs (azathioprine, steroids and antilymphocyte globulin) patients are supposed to be susceptible to skin and lip cancers (40%) whereas the recently introduced new immunosuppressive drug cyclosporine A shows a comparable incidence of non-Hodgkin lymphomas (41%) and the patients seem to develop more potentially life threatening tumours of internal organs. In the present paper we report on a case of laryngeal carcinoma following immunosuppressive treatment with cyclosporine A 10 months after heart transplantation.
