Zusammenfassung
Hintergrund: Hepato-portomesenteriale Lufteinschlüsse können im Rahmen schwerer intraabdomineller Infektionen auftreten. Da sie mit einer hohen Letalität assoziiert sind, gelten sie als alarmierendes Zeichen.
Kasuistiken: Bei einem 65-jährigen Patienten kam es im Rahmen einer fortgeschrittenen perforierten Sigmadivertikulitis zum Auftreten von hepato-portomesenterialen Lufteinschlüssen, die im Rahmen der CT-Diagnostik nachgewiesen wurden. Bei einem 75-jährigen Patienten trat dieses Phänomen im Rahmen einer Inkarzeration eines Upside-down-Magens auf. Beide Patienten wurden unverzüglich operiert und überlebten die schwere Erkrankung.
Schlussfolgerung: Da die Ursache für hepato-portomesenteriale Lufteinschlüsse in der Regel schwerwiegende intraabdominelle Erkrankungen mit hoher Letalität sind, sollte (spätestens) dieses „Alarmsignal” die umgehende Laparotomie zur Folge haben, wenn die Indikation nicht ohnehin aus anderen Gründen bereits gegeben ist.
Abstract
Background: Hepatic portal venous gas, which may occur in conjunction with severe intra-abdominal infections, is associated with a high mortality rate and is considered as an alarm signal.
History: CT diagnostics revealed hepatic portal venous gas in a 65-year-old patient with advanced perforated sigmoid diverticulitis. This phenomenon was also observed in a 75-year-old patient in association with an upside-down stomach incarceration. Both patients underwent emergency surgery and survived this life-threatening disease.
Conclusion: Since hepatic portal gas is usually caused by severe intra-abdominal diseases with a high mortality rate, this warning sign should, at the latest, result in urgent laparotomy, if the indication has not already been established for other reasons.
