Die Grenzen der kieferorthopädischen Behandlung und der Camouflage-Behandlung bei Klasse-III-Dysgnathien

Zusammenfassung

Patienten mit einer skelettalen Klasse III können entweder kieferorthopädisch, mit einer kieferorthopädischen Camouflagebehandlung oder kombiniert kieferorthopädisch / kieferchirurgisch behandelt werden, was von dem Grad der skelettalen Diskrepanz, dem skelettalen Muster und dem Alter des Patienten abhängt. Eine rein orthopädische Behandlung kommt nur für Kinder in Betracht, die sich noch im Wachstum befinden und die somit noch von einer Wachstumsänderung profitieren können. Die Behandlung mit maxillärer Vorverlagerung ist bei Klasse-III-Patienten mit einer unterentwickelten Maxilla und einem relativ normal entwickelten Unterkiefer effektiv. Nicht angemessen ist dieses Vorgehen für Patienten mit überentwickeltem Unterkiefer und solche mit exzessivem Unterkieferwachstum. Camouflage-Behandlungen können bei Patienten durchgeführt werden, die eine milde Form der skelettalen Klasse III aufweisen und bei denen kein Restwachstum vorhanden ist. Die Behandlung kann mit Extraktion der unteren Prämolaren, der zweiten Molaren, eines Schneidezahns oder sogar mithilfe von Minischrauben erfolgen. Allerdings ist bei Patienten mit einer schweren skelettalen Diskrepanz oder einem kontinuierlichen Unterkieferwachstum eine kombiniert kieferorthopädische / kieferchirurgische Therapie notwendig.

Abstract

Skeletal Class III patients can be treated by either orthopedics, orthodontic camouflage, or orthognathic surgery, depending on the degree of skeletal discrepancy, the skeletal pattern, and the age of the patient. The orthopedic approach for growth modification is usually limited to children with growth remaining. Treatment by maxillary protraction is effective in Class III patients with an underdeveloped maxilla and a fairly normal mandible. However, this approach may not be suitable for patients with an overdeveloped mandible or a mandible that continues to grow excessively. Camouflage orthodontic treatment may be performed in patients with a mild skeletal Class III discrepancy and no remaining growth by extracting lower premolars, second molars, incisors, or even using mini-implants. However, in patients with a severe skeletal discrepancy or continuous mandibular growth, it is necessary to consider a combined surgical/orthodontic approach.
