Zusammenfassung
Grundlagen: Eine biliodigestive Anastomose mit Roux-en-Y-Rekonstruktion macht eine konventionelle retrograde Cholangiografie mit Standard-Duodenoskopen aufgrund der Länge des zwischengeschalteten Dünndarmsegments so gut wie unmöglich. Die Doppelballon-Enteroskopie (DBE) ermöglicht eine tiefe Insertion in den Dünndarm und könnte bei diesen Patienten eine ERC ermöglichen. In dieser Studie berichten wir über unsere Erfahrungen mit der diagnostischen und therapeutischen DBE-ERC bei Patienten mit biliodigestiver Anastomose mit langer Roux-en-Y-Schlinge. Bei allen Patienten war zuvor ein Versuch der ERC mit Standardendoskopen frustran gewesen. Patienten und Methoden: Zwischen Dezember 2004 und Mai 2008 wurden bei 15 Patienten (mittleres Alter 60,2 Jahre) mit biliodigestiver Anastomose insgesamt 25 DBE-ERC-Prozeduren durchgeführt. Ergebnisse: Eine Kannülierung der Gallenwege wurde bei 22 von 25 Prozeduren erreicht (84 %). 21 therapeutische Interventionen (Gallensteinextraktionen, Gallenwegsdilatationen sowie Einlage und Entfernung von Stents) wurden bei 16 DBE-ERCs bei 8 Patienten durchgeführt. Die mittlere Untersuchungszeit pro Prozedur war 74,6 ± 25,0 Minuten. Postinterventionell kam es nach 4 Prozeduren bei 3 Patienten mit Cholangitis zu selbstlimitierten Fieberschüben. Nach therapeutischer DBE-ERC kam es bei allen Patienten zu einem signifikanten Abfall des Bilirubins und alle außer einem Patienten wurden beschwerdefrei (Nachbeobachtung für 10,4 ± 8,6 Monate). Schlussfolgerung: Die DBE-Technik ermöglicht eine diagnostische und therapeutische ERC in Patienten, bei denen zuvor der endoskopische Zugang zum Gallenwegssystem bedingt durch operativ veränderte Anatomie des oberen Gastrointestinaltrakts nicht möglich war. Die DBE-ERC ist ein sicheres Verfahren mit hoher Erfolgsrate und günstigem Einfluss auf den Krankheitsverlauf.
Abstract
Background: Choledochojejunal anastomoses with Roux-en-Y reconstruction excludes the biliary tract from conventional endoscopic retrograde cholangiography (ERC) with standard endoscopes due to the length of the interposed small bowel segment. Double-balloon enteroscopy (DBE) facilitates deep insertion into the small bowel and may be used to perform ERC in these patients. In the present case series we report our experience with diagnostic and therapeutic double-balloon ERC in patients with choledochojejunostomy to a long Roux-en-Y loop previously unavailable for standard length endoscopes. Patients and Methods: Between December 2004 and May 2008 15 patients (mean age: 60.2 years) with choledochojejunal anastomosis underwent a total of 25 DBE-ERC procedures. Results: Cannulation of the bile ducts was achieved in 22 / 25 procedures (84 %). Twenty-one therapeutic interventions, including stone removal, biliary duct dilation, stent placement and removal of previously placed stents were performed during 16 procedures in 8 patients. The mean total duration time of the procedures was 74.6 ± 25.0 minutes. Postinterventional self-limiting fever occurred after 4 procedures in 3 patients with cholangitis. After therapeutic interventions all patients had a significant drop of bilirubin levels and all except one patient were free of complaints (follow-up 10.4 ± 8.6 months). Conclusions: The DBE system permits diagnostic and therapeutic ERC in surgically modified anatomy, previously unavailable for endoluminal access. In our experience this procedure is safe and has a high success rate with a favourable patient outcome.
