Pneumothorax beim Intensivpatienten: Die Wertigkeit von Tangentialaufnahmen

Zusammenfassung

Bei 55 Tntensivpatienten wurde bei akuter Verschlechterung des Gasaustausches ohne nachweisbaren Pneumothorax, bei suspektem Pneumothoraxbefund oder bei nachgewiesen schmalem Pneumothorax zusätzlich zum a. p. Lungenbettröntgen eine Tangentialaufnahme angefertigt. In 14 von 42 Fällen (33,3 %) mit negativem oder suspektem Bettlungenröntgen wurde ein Pneumothorax nachgewiesen, wobei es sich bei 6 von diesen 14 Fällen um einen Spannungspneumothorax handelte. Bei 7 von 11 Fällen (63,3 %) mit schmalem Pneumothorax am a.p. Lungenröntgen zeigte die Tangentialaufnahme zusätzlich Spannungszeichen.

Summary

In 55 intensive-care patients an additional tangential view of the chest was taken to demonstrate or exclude a pneumothorax in patients with sudden deterioration of gas exchange and negative ap-chest x-ray, if there was a suspicion of pneumothorax or a confirmed small pneumothorax in the ap-view. In 14 of 42 cases (33.3 %) with negative or suspected ap-chest x-ray the tangential view revealed a pneumothorax. 6 of these 14 pneumothoraces were under tension. In 7 out of 11 patients (63.6 %) with small pneumothorax, the tangential view showed additionally a tensionpneumothorax.
