Abstract
The present article deals with the determination of the lymphocyte subpopulations as well as of the alpha-fetoprotein serum concentrations in premature and newborn, and examines their dependence on the periof of gestation. Despite significantly increased total lymphocyte values in newborn, compared with older children, no significant increase of the subpopulation counts can be found. The relative values of the T, B-lymphocytes, as well as IgG and IgA- receptors on the B-lymphocytes, are significantly lower in premature babies and in newborn compared with older children. A comparison of the group of infants aged 12-32 days with the group of premature babies and newborn shows a significant increase in the relative values of the T-lymphocytes, B-lymphocytes, as well as the lymphocytes with the immunoglobulin receptors IgG and IgA.
Since the increase of lymphocytes with the IgA receptors was particularly great, the influence of mechanical respiration and of nasal CPAP its also discussed. Determination of the alpha-fetoprotein serum concentrations showed a significant drop with increasing period of gestation of the newborn. Determination of the period of gestation on the basis of the alpha-fetoprotein serum concentrations, however, does not appear possible in view of the wide range of variation of the individual results.
The low relative values of the lymphocyte subpopulations with high alpha-fetoprotein serum concentrations cannot be described alone by means of an immunosuppressive action of the alpha-fetoprotein, as described by Gupta. We are rather inclined to believe that mutual influencing is simulated by the dependence of the lymphocyte subpopulations and the alpha-fetoprotein serum concentrations on the period of gestation of the newborn.
Zusammenfassung
Diese Arbeit befaßt sich mit den Bestimmungen der Lymphozytensubpopulationen, sowie der α1 -Fetoproteinserumkonzentration bei Früh- und Neugeborenen und untersucht ihre Abhängigkeit zum Gestationsalter. Trotz signifikant höherer Gesamtlymphozytenwerte bei Neugeborenen im Vergleich zu älteren Kindern ist bei ihnen keine signifikante Erhöhung der Subpopulationen nachweisbar. Die Relativwerte der T-, B-Lymphozyten, sowie IgG- und IgA-Rezeptoren auf B-Lymphozyten sind bei Früh- und Neugeborenen im Vergleich zu älteren Kindern signifikant erniedrigt. Ein Vergleich der Gruppe von Säuglingen im Alter von 12-32 Tagen mit der Gruppe der Früh- und Neugebprenen ergibt einen signifikanten Anstieg der Relativwerte der T-Lymphozyten, B-Lymphozyten, sowie der Lymphozyten mit den Immunglobulinrezeptoren IgG und IgA. Da der Anstieg der Lymphozyten mit den IgA-Rezeptoren besonders stark war, wurde hier der Einfluß der maschinellen Beatmung und des nasalen CPAP diskutiert.
Die Bestimmung der Alpha-, Fetoprotein-Serumkonzentration ergab einen signifikanten Abfall der Werte mit steigendem Gestationsalter des Neugeborenen. Eine Bestimmung des Gestationsalters aufgrund der Alpha-Fetoprotein-Serumkonzentration erscheint uns wegen der großen Variationsbreite der Einzelergebnisse jedoch nicht möglich.
Die niedrigen Relativwerte der Lymphozytensubpopulationen bei hohen Alpha-, Fetoproteinserumkonzentrationen lassen sich nicht allein durch eine von Gupta (1, 42) beschriebene immunsuppressive Wirkung des Alpha-, Fetoproteins erklären. Vielmehr scheint durch die Abhängigkeit der Lymphozytensubpopulationen und Alpha-, Fetoprotein- Serumkonzentrationen vom Gestationsalter des Neugeborenen eine gegenseitige Beeinflussung vorgetäuscht zu werden.
