Zusammenfassung
Hintergrund Infektionen der Hornhaut, sogar mit Beteiligung intraokularer Gewebe, stellen schwerwiegende Komplikationen nach photorefraktiver Keratektomie dar. Es werden drei Patienten beschrieben.
Patienten und Methode Nach 39 photo-therapeutischen und 118 photorefraktiven komplikationslosen Eingriffen kam es bei vier jungen Behandelten zu Hornhautinfiltrationen, wobei sich bei drei Patienten ein Hornhautgeschwür und bei einem Patient eine Endophthalmitis entwickelten. Verwendet wurde das Excimer-Laser-Gerät Keratom der Fa. Schwind (193 nm Wellenlänge, 10 Hz, Energiedichte 180-200 mJ/cm2 ). Gemeinsam war bei allen die unmittelbare postoperative Applikation einer Einmalkontaktlinse, welche über Nacht getragen wurde.
Therapie und Verlauf Da sich im Abkratzmaterial der Hornhautgeschwüre bei allen drei Patienten Pilzhyphen nachweisen ließen, wurde eine Therapie mit lokalen und allgemeinen Antimykotika durchgeführt. Aufgrund des örtlichen und zeitlichen Zusammentreffens wurde auch der Patient mit der Endophthalmitis neben Antibiotika mit Antimykotika behandelt. Alle Hornhautulzera heilten mit einer Narbe ähnlich einem Haze mit Grad 3-4 aus, das Auge mit der Endophthalmitis normalisierte sich beinahe vollständig.
Schlußfolgerung Möglicherweise bewirkte das Tragen der Kontaktlinse während des sehr heißen und feuchten Sommers nach der „Traumatisation” der Hornhaut durch den Excimer-Laser bei bekannt höheren Schimmelpilzkonzentrationen in der Luft die Ausbildung von Hornhautgeschwüren. Der Verzicht auf die postoperative Applikation von Kontaktlinsen, besonders das Vermeiden des Tragens über Nacht, werden empfohlen.
Summary
Background Infections of the cornea with or without involvement of intraocular tissues are severe complications after photorefractive keratectomy. We report on three cases with ulcerative keratitis and one case with severe endophthalmitis, which developed some days after laser treatment.
Patients and methods After 39 photo-therapeutic and 118 photo-refractive treatments without complications four young treated persons suffered from corneal infiltrations. Three of them developed severe corneal ulcerations, one a severe endophthalmitis. We used the laser machine of the company Schwind (193 nm wavelenghth, repetition rate 10 Hz, fluence 180-200 mJ/cm2 ). One part of the treatment which all patients had in common was the application of a disposable contact lens postoperatively, which was worn overnight.
Therapy and follow-up Because all three patients with ulcerations revealed hyphen of fungi in their cornealscratch material they were treated with antifungal agents locally and systemically. Due to the local and temporal connection the patient with the endophthalmitis was treated with antifungal agents, too. All corneal ulcerations healed with scars similar to a haze graded 3 to 4, the eye with the endophthalmitis healed almost completely.
Conclusion Probably the use of disposable contactlenses postoperatively, especially the overnight wear, during an extremly hot and humid summer lead to the described severe infections of the eyes after the “traumatisation” of the cornea by the excimer laser. A known higher concentration of Aspergillus in the air due to renovations of buildings in the clinical area might have been an additional negative effect. The avoidance of the use of disposable contact lenses postoperatively, especially the overnight wear, is emphazised.
