Zusammenfassung
Das SCC (Squamous Cell Carcinoma)-Antigen ist eine Fraktion des aus Plattenepithelkarzinomen der Cervix uteri gewonnenen Tumorantigens TA-4. In einer retrospektiven Studie wurde die klinische Bedeutung des SCC-Antigens an Seren von 119 Kontrollpersonen, 30 Patientinnen mit zervikalen intraepithelialen Neoplasien (CIN I-III), 170 Frauen mit Zervixkarzinom und 82 Patientinnen mit anderen malignen gynäkologischen Tumoren untersucht. Die radioimmunologische Bestimmung erfolgte mit einem Kit der Firma Abbott. Als Normbereichsgrenzwert wurde 2,5 ng/ml festgelegt.
Erhöhte Serumkonzentrationen des SCC-Antigens fanden sich bei 5% der Blutspenderinnen, 3% der Patientinnen mit Uterus myomatosus und 13% der Frauen mit CIN I-III. Patientinnen mit primärem oder rezidiviertem Plattenepithelkarzinom der Zervix wiesen in 62% bzw. 73% pathologische SCC-Antigenkonzentrationen auf. Nur eine von elf Patientinnen mit einem primären oder rezidivierten Adenokarzinom der Zervix hatte einen geringgradig erhöhten Antigenspiegel. Die Positivitätsrate war von der Ausdehnung der Plattenepithelkarzinome der Zervix abhängig und stieg von 32% im FIGOStadium I auf 83% im Stadium III/IV an. Nach erfolgreicher Primärbehandlung eines Zervixkarzinoms rezidivfreier Patientinnen wiesen nur 2% der Fälle SCC-Antigenkonzentrationen von über 2,5 ng/ml auf. Die Positivitätsraten betrugen 33% beim primären Vulva- und Vaginalkarzinom sowie 8% bzw. 15% beim primären Endometrium- oder Ovarialkarzinom. Frauen mit einem primären Mammakarzinom hatten in keinem Fall Serumspiegel von über 2,5 ng/ml. In der Verlaufskontrolle (1 - 36 Monate) von 29 Patientinnen mit primärem Plattenepithelkarzinom der Zervix korrelierten die Serumkonzentrationen in 18 Fällen mit dem Krankheitsgeschehen. Aufgrund der hohen Sensitivität und Spezifität sowie der Korrelation zur Tumoraktivität in der Verlaufsbeobachtung erscheint die routinemäßige Bestimmung des SCC-Antigens zur Kontrolle von Patientinnen mit Plattenepithelkarzinomen der Zervix empfehlenswert.
Abstract
SCC (Squamous Cell Carcinoma) antigen is a fraction of the tumor antigen TA-4, obtained from squamous cell carcinomas of the cervix uteri. In a retrospective study the clinical significance of SCC antigen was investigated in sera of 119 controls, 30 patients with cervical intraepithelial neoplasia (CIN I-III), 170 women with cervical carcinoma, and 82 patients with other malignant gynecological tumors. Radioimmunoassay was performed with a kit manufactured by Abbott Diagnostics. The limit of the normal range was 2.5 ng/ml.
Elevated serum concentrations of SCC antigen were measured in 5% of blood donors, 3% of patients with uterus myomatosus, and 13% of women with CIN I-III. Pathologic SCC antigen concentrations were found in 62% of patients with primary and 73% of women with recurrent cervical squamous cell carcinomas. Only one out of eleven patients with a primary or recurrent adenocarcinoma of the cervix had a slightly elevated antigen level. The positivity rates depended on the spread of the cervical squamous cell carcinomas of the cervix and rose from 32% at FIGO stage I to 83% at stages III/IV. Only 2% of the patients with no evidence of recurrent disease after successful primary treatment of a cervical carcinoma had SCC antigen concentrations exceeding 2.5 ng/ml. The positivity rates were 33% in cases of primary vulval and vaginal carcinomas, 8% in primary endometrial carcinoma, and 15% in primary ovarian carcinoma. None of the women with primary breast cancer had a serum level above 2.5 ng/ml. In the followup (1 - 36 months) of 29 patients with primary squamous cell carcinoma of the cervix the serum concentrations correlated with the course of disease in 18 cases. Owing to its high sensitivity and specificity, and to the correlation with tumor activity during the observation period, routine determination of SCC antigen appears recommendable for monitoring patients with squamous cell carcinomas of the cervix.
