Meningeosis carcinomatosa unter Cis-Platin-Therapie bei gynäkologischen Karzinomen

Zusammenfassung

Es wird über den klinischen Verlauf von zwei Patientinnen mit einem fortgeschrittenen Ovarialkarzinom bzw. Zervixkarzinom mit Meningeosis carcinomatosa unter einer Cis-Platin-Kombinationsbehandlung (PAC-Schema) berichtet. Beide Patientinnen waren vor dem Einsatz der Chemotherapie neurologisch unauffällig. Die ZNS-Symptomatik entwickelte sich sehr rasch nach Applikation geringer CIS-Platin-Dosen (Gesamtdosis: 160 bzw. 300 mg) und ließ sich therapeutisch nicht mehr beeinflussen. In einem Fall konnten im Liquor Tumorzellen festgestellt werden.

Die Todesursache könnte Folge eines ungewöhnlichen toxischen Effektes von Cis-Platin gewesen sein. Eine sorgfältige neurologische Untersuchung sollte deshalb vor und unter dem Einsatz einer Chemotherapie mit Cis-Platinum zur möglichst rechtzeitigen Erkennung außerordentlicher toxischer Nebenwirkungen erfolgen.

Abstract

Two cases of carcinomatous meningeosis are reported. One occurred during cis-platinum combination treatment (PAC-scheme) for advanced ovarian carcinoma and one during the same treatment for a carcinoma of the cervix. Both patients showed no neurological problems prior to chemotherapy. The central nervous symptoms developed rapidly following the administration of small doses of cisplatinum (total dose 160 or 300 mg) and could not be controlled. In one case tumour cells were found in the cerebral spinal fluid.

The cause of death could have been an unusually toxic reaction to cis-platinum. A thorough neurological examination prior and during chemotherapy with cis-platinum is necessary in order to recognize toxic side effects of the drug as soon as possible.
