Messung der motorischen Nervenleitgeschwindigkeit zur präzisen Reifebestimmung bei untergewichtigen Neugeborenen

Zusammenfassung
In der Zeit vom 1.10.1979 bis 31.1.1981 wurden an der Abteilung für Geburtsmedizin des Krankenhauses Ber lin-Neukölln bei 379 eutrophen und bei 78 hyportrophen Neugeborenen Reifebestimmungen mit Hilfe der motorischen Nervenleitgeschwindigkeit (NLG) in Kombination mit dem Farr-Schema durchgeführt. Wie bei den eutrophen Kindern stieg der Median der Nervenleitgeschwindigkeit für die hypotrophen Kinder von 16,5 m/sec in der 34. Schwangerschaftswoche bis zu 25,7 m/sec in der 41. Schwangerschaftswoche nahezu linear an. Die Signifikanzberechnung für die Nervenleitgeschwindigkeit beider Gruppen ergab keinen Unterschied (p <0,01). Die Reifebestimmung mit dem Farr-Schema wich hingegen bis zur 38. Schwangerschaftswoche stark in falsch positiver Richtung ab.
Die Messung der motorischen Nervenleitgeschwindigkeit des N. ulnaris erwies sich als einfache und sehr präzise postpartuale Gestationsaltersbestimmung, die eine exakte Differenzierung untergewichtiger Kinder in hypotrophe Neugeborene und eutrophe Frühgeborene ermöglicht.
Summary
Between 1.10.1979 and 31.1.1981, maturity estimation tests were conducted on 379 eutrophic and 78 hypotrophic neonates at the Department of Obstetrics in Berlin Neukölln by means of motor nerve conduction velocity (NCV) combined with the FARR system. In both eutrophic and hypotrophic infants the median of the nerve conduetion velocity rises almost in a straight line from 16.5 m/sec in the 34th week of gestation to 25.7 m/ sec in the 41st week. The significance calculation of the nerve conduetion velocity of both groups showed no variation (p <0,01). On the other hand the maturity estimation made with the FARR system deviated strongly in a false positive direction up to the 38th week of gestation.
The measurement of the motor nerve conduetion velocity of the ulnar nerve has proved to be a simple and very precise estimation of the Postpartum gestational age which makes it possible to distinguish with certainty between premature eutrophic infants and hypotrophic infants of low birthweight.
