Langzeitergebnisse mit der Wagner-Metall/Metall-Schalenprothese

Zusammenfassung

Einleitung: Frühere Fehlschläge des Oberflächenersatzes des Hüftgelenks in den 1980er-Jahren wurden der Verwendung ungeeigneter Gleitpaarungen und einer nicht optimalen Platzierung der Implantate zugeschrieben. Durch die Entwicklung von Metall/Metall-Gleitpaarungen Anfang der 90er-Jahre wurde versucht, diese beiden Probleme zu lösen. Ziel dieser Arbeit ist die Darstellung der Langzeitergebnisse der Wagner-Schalenprothese als Metall/Metall-Gleitpaarung. Patienten und Methode: Von 1/1991 bis 12/1995 erfolgten 40 Implantationen einer Schalenprothese nach Wagner. Alle Operationen wurden über einen vorderen Zugang durchgeführt. 37 Patienten konnten bis 12/2006 nach durchschnittlich 13,1 Jahren nachuntersucht werden. Zur Beurteilung des klinischen Ergebnisses wurde der Score nach Merle d'Aubigné verwendet. Zur Beurteilung der radiologischen Ergebnisse wurden Röntgenaufnahmen des Hüftgelenkss in 2 Ebenen durchgeführt. Ergebnisse: Durchschnittlich wurden bei jedem Patienten vor der eigentlichen Schalenprothesenimplantation 1,45 (1 - 9) Voroperationen durchgeführt. In 32 Fällen wurde eine zementfreie femorale Komponente der Schalenprothese verwendet, in 8 Fällen eine zementierte. Der Score nach Merle d'Aubigné verbesserte sich von präoperativ durchschnittlich 8,3 auf postoperativ durchschnittlich 17,3 Punkte (p < 0,001). 17 der 40 (42,5 %) Schalenprothesen wurden revidiert. Diskussion: Bei kritischer Betrachtung der Ergebnisse ist die relativ hohe Rate an Revisionen größtenteils auf die extreme Indikationsstellung, welche heute nicht mehr dem Standard entspräche, zurückzuführen. Die Implantation eines Metall-Metall-Oberflächenersatzes am Hüftgelenk bei strenger Indikationsstellung sowie exakter und sorgfältiger Operationstechnik bietet dennoch eine Möglichkeit, gerade jüngere Patienten mit einer Hüftprothese zu versorgen, die auch bei Versagen der Implantate Rückzugsmöglichkeiten bietet.

Abstract

Introduction: Former failures of hip resurfacing arthroplasties in the 1980s were attributed to articulating materials and malpositioning of the implants. To solve these problems in the 1990s metal-on-metal articulations were developed. The purpose of this study was to depict the long-term results of the Wagner resurfacing prosthesis utilising a metal-on-metal surface articulation. Patients and Method: Between January 1991 and December 1995 40 Wagner resurfacing prostheses were implanted. All operations were performed via an anterior approach. 37 patients were able to be followed up after an average period of 13.1 years. For clinical evaluation the score according to Merle d'Aubigné was used. For radiological assessment X-rays in two planes were evaluated. Results: On average, 1.45 (1 to 9) preliminary operations were performed on each patient prior to the implantation of the actual surface replacement prosthesis. In 32 cases a cementless femoral component was used, in the remaining 8 cases a cemented one. The score according to Merle d'Aubigné improved from a pre-operative average of 8.3 to a post-operative average of 17.3 points (p < 0.001). 17 of 40 (42.5 %) hip resurfacing arthroplasties have been revised. Discussion: Taking all aspects into account, the high rate of revisions in this series was attributed to extreme indications for hip resurfacing which would not be considered as standard nowadays. Combined with a stricter indication and an exact, careful operative technique, the metal-on-metal surface replacement of the hip joint may offer an alternative replacement option.
