Epiphyseolysis capitis femoris: Überlegungen zur Therapie der Gegenhüfte am Wachstumsende

Zusammenfassung

Allgemein ist die prophylaktische Behandlung der Gegenhüfte bei einseitig manifester E. c. f. eine bewährte Präventivmaßnahme. Kurz vor Wachstumsende kann jedoch aufgrund eines asynchronen Fugenverschlusses die Gegenseite bereits fusioniert sein, wodurch die Gefahr eines Abrutsches für diese Hüfte nicht mehr gegeben ist. Diagnostische Schwierigkeiten bei der radiologischen Beurteilung der Fugenstruktur können mit dem CT gelöst werden. In einem Zeitraum von 2 Jahren konnte bei 3 Fällen (von 9) ein Eingriff auf der kontralateralen Seite aufgrund des im CT gesicherten Fugenverschlusses vermieden werden.

Abstract

In general, prophylactic treatment of the contralateral hip in manifest unilateral EFH is a wellproven and efficacious preventive measure. Shortly before the end of growth, however, due to asynchronous closure of the growth plate, the contralateral side may already be fused, so that there is no longer any danger of the hip slipping. Diagnostic difficulties in radiologic assessment of growth plate structure can be resolved with CT. In a two-year period, CT confirmation of closure of the growth plate enabled surgery on the contralateral side to be dispensed with in 3 cases (out of a total of 9).
