Operative Therapie und Ergebnisse bei Rotatorenmanschettenruptur

Zusammenfassung

Die Ergebnisse einer Retrospektivuntersuchung von 57 an einer Rotatorenmanschettenruptur operierten Patienten werden analysiert und im Hinblick auf Literaturangaben diskutiert. Operationstechnisch sind ein kurzer ventraler Zugang und die transossäre Refixation nach McLaughlin Methode der Wahl. Die Acromioplastik nach Neer ist als Routinemaßnahme nicht indiziert. Adhäsionen im subcoracoidalen Raum müssen beachtet und ggf. gelöst werden. Als prognostisch günstig hat sich neben einem höheren Lebensalter eine kurze Anamnesedauer erwiesen. Ein langer Nachuntersuchungszeitraum verbessert die Ergebnisse weiterhin.

Abstract

Follow up evaluations of 57 patients with surgically treated rotator cuff tears are presented and valued with regard to the literature. A short anterior exposure and cuff anchorage to the humerus is the method of choice. Anterior ancromioplasty should not be a routine. Attention should be paid to adhesions of the subcoracoidal space, they should be separated if necessary. Patients being of older age, with a short history and a long time interval between operation and final evaluation do have the best results.
