Abstract
Due to the high cost involved, it has often been discussed in the past to exclude topical NSAIDs for rheumatoid diseases from reimbursement. Therefore, we conducted a comparative cost analysis to get some information on the cost of therapy of one episode of osteoarthritis of knee and/or finger. 154 office-based physicians (GPs, specialists in internal medicine, orthopaedic surgeons, rheumatologists) documented the data of 1525 patients treated with either a topical or systemic NSAID due to an episode of osteoarthritis of the knee and/ or finger. Three different therapeutic regimes were identified: (1) Therapy with a topical NSAID only (458 patients), (2) therapy with oral NSAIDs only (564 patients), (3) topical as well as systemic therapy with NSAID (503 patients). We analysed the treatment costs from the perspective of the payers (sickness funds). The mean costs per patient per episode of osteoarthritis of the finger and/or the knee were DM 497.16. The mean treatment costs differentiated according to the treatment groups were DM 216.47 per patient (1st group), DM 592.28 per patient (2nd group), DM 646.08 per patient (3rd group). High cost items in group 2 and 3 were inpatient costs due to osteoarthritis and due to side effects of the systemic therapy. The consequences of excluding topical NSAR from reimbusement is discussed from the perspective of social health insurance bodies.
Zusammenfassung
In den vergangenen Jahren wurde vermehrt über den therapeutischen Nutzen der topischen NSAR-Therapie bei rheumatischen Erkrankungen vor allem unter Kostenaspekten diskutiert. Aus diesem Grunde wurde eine retrospektive Kostenanalyse im Indikationsgebiet aktivierte Gon- oder Fingerarthrose durchgeführt. 154 niedergelassene praktische Ärzte, Allgemeinärzte, Internisten, Orthopäden und Rheumatologen dokumentierten von 1525 Patienten die Therapie eines akuten arthrotischen Schubs. Die Patienten wurden dabei entweder topisch mit Diclofenac als Emulgel oder einem beliebigen oralen NSAR behandelt. In der Analyse konnten folgende Therapieschemata identifiziert werden: 1. ausschließlich Emulgel (458 Patienten), 2. ausschließlich orale NSAR-Therapie unabhängig vom Wirkstoff (564 Patienten) und 3. sowohl orale NSAR- als auch topische Emulgel-Therapie mit einem NSAR (503 Patienten). Aus der Perspektive der Gesetzlichen Krankenversicherung wurden die anfallenden Behandlungskosten ermittelt. Dabei ergaben sich Durchschnittskosten pro Patienten von 497.16 DM pro akutem Arthroseschub. Differenziert nach Behandlungsgruppen fanden sich folgende Durchschnittskosten: 216.47 DM pro Patient (1. Gruppe), 592.28 DM pro Patient (2. Gruppe), 646.08 DM pro Patient (3.Gruppe). Überwiegend verantwortlich für die Kosten in den Gruppen 2 und 3 waren Krankenhausaufenthalte wegen der Grunderkrankung oder wegen Nebenwirkungen aufgrund der oralen Therapie. Es wird diskutiert, welche Auswirkung der Verzicht auf topische NSAR auf die Kostensituation der Gesetzlichen Krankenversicherer hat.
