Abstract
Reports on the surgical treatment of psoriatic arthritis are rare. We studied the different indications for surgery, the complications which occurred and classified the patients into different disease patterns of psoriatic arthritis. We analysed 23 female and 24 male patients with psoriatic arthritis according to Moll and Wright who were operated on at two centers from 1971 to 1998. A total of 117 operative interventions was performed at a mean age of 48 y. (range 20 - 81 y.). Preventive surgery accounted for more than 50% of the operative procedures. Eight open and 21 arthroscopic knee synovectomies were carried out. Twenty-two finger joint synovectomies and 13 finger tenosynovectomies were performed. Reconstructive surgery included 17 resection arthroplasties, 11 arthrodeses and 15 joint replacements. Ten other interventions were carried out. Complications observed were three delayed wound healings (2 forefoot, 1 finger) which ended uneventfully, one finger flexor tendon adhesion which had to be revised and four recurrences of knee synovitis, three of which had to have a second synovectomy. No infection was observed. More than 60% of patients had an oligoarthritis, about 30% had a polyarthritis. Patients with a knee synovectomy more often had an oligoarthritis whereas hand surgery was performed more often in polyarthritic patients. The operative procedures in psoriatic arthritis are similar to those in rheumatoid arthritis. The disease pattern of psoriatic arthritis influences, in combination with other parameters, the indication and type of the surgical procedure. Therefore, the disease pattern should be observed in all studies. Knee synovitis has a relatively high recurrence rate and needs to be supplemented by other therapies such as radiation synovectomy and second line medication. Infection was not observed. A recognized classification and core data of psoriatic arthritis need to be established. Subsequently, prospective multicentre studies can answer open questions of surgical therapy.
Zusammenfassung
Untersuchungen über die operative Therapie der Psoriasisarthritis sind selten. In dieser Arbeit werden die Operationsindikationen dargelegt, die Komplikationen aufgezeigt und die Patienten klinisch in verschiedene Psoriasisarthritistypen klassifiziert. Über Zeiträume von 14 bzw. 27 Jahren wurden retrospektiv an zwei Zentren 23 weibliche und 24 männliche Patienten mit gemäß Moll und Wright gesicherter Psoriasisarthritis gefunden, an denen 117 operative Eingriffe vorgenommen wurden. Präventive Eingriffe machten mehr als die Hälfte der Operationen aus. Acht offene und 21 arthroskopische Kniesynovektomien sowie 22 Fingergelenk- und 13 Tenosynovektomien wurden durchgeführt. Rekonstruktive Eingriffe umfaßten 17 Resektionsarthroplastiken, 11 Arthrodesen und 15 Prothesenimplantationen. Zehn Eingriffe fallen unter andere Kategorien. Komplikationen waren drei sekundäre Wundheilungen, eine operativ revidierte Sehnenverklebung und vier Rezidivsynovitiden am Knie, von denen drei reoperiert wurden. Es traten keine Infektionen auf. Mehr als 60% der Patienten hatten eine Oligoarthritis, knapp 30% eine Polyarthritis. Oligoarthritiden überwogen bei Kniesynovektomien, Polyarthritiden bei Eingriffen an der Hand. Die operativen Behandlungsverfahren der Psoriasisarthritis entsprechen denen der Rheumatoiden Arthritis. Die Psoriasisarthritistypenzugehörigkeit beeinflußt mit anderen Parametern die Indikationsstellung und die Verfahrenswahl und sollte in allen Studien festgehalten werden. Kniesynovektomien haben eine relativ hohe Rezidivrate und bedürfen der Ergänzung durch andere Therapieverfahren. Die in der Literatur erwähnte erhöhte Infektionsrate wurde nicht beobachtet. Nach Erstellen einer verbindlichen Klassifikation und Festlegen von Kerndaten der Psoriasisarthritis sind neue prospektive und multizentrische Studien der operativen Therapie notwendig.
