Abstract
The rheumatological data base of the German collaborating Rheumatic Diseases Centres yields information on the quality of health care received by patients suffering from inflammatory rheumatic diseases. The main quality indicators are: percentage of rheumatoid arthritis patients in the population seen by a rheumatologist; medical history concerning second-line antirheumatic drugs; present therapies; frequency and duration of inpatient treatment; adequacy of rehabilitative treatment; and health outcomes such as patient's global assessment of well-being or functional status. Even now, patients with inflammatory rheumatic diseases consult a rheumatologist at a rather late stage of their disease (mean disease duration in RA: two years). There are considerable differences in the therapeutic styles of rheumatologists compared with general practitioners: 31 % of the patients treated by general practitioners had been treated with second-line antirheumatic drugs in the previous year, compared to 86% of the patients treated by rheumatologists. Rheumatologists prescribe more active physiotherapies, technical aids and rehabilitative measures. Within three observed years (1993-1995), the percentage of patients with RA who had inpatient hospital treatment during one year was reduced from 26.5% to 17.2%. The duration of hospitalisation was also reduced even though 25% of the patients still remain in hospital for more than four weeks. More than 14% of the patients had been treated in rehabilitation clinics during the preceding one-year period. Patients with rheumatoid arthritis in continuous rheumatological care are more active in their home exercises, and they report on better subjective well-being. Over the course of two years, their functional status remains almost unchanged.
Zusammenfassung
Die Daten der Kerndokumentation der Arbeitsgemeinschaft der regionalen kooperativen Rheumazentren ermöglichen es, über verschiedene Indikatoren die Qualität der ambulanten rheumatologischen Versorgung zu beurteilen. Diese beziehen sich auf den Erreichungsgrad der Kranken in der Bevölkerung, die Versorgungsanamnese, auf Umfang und Art der medikamentösen und ergänzenden Therapie sowie die Entwicklung wichtiger Outcomes (z.B. Funktion, subjektiver Gesundheitszustand). Nach wie vor kommen Patienten mit entzündlich-rheumatischen Krankheiten nur zum kleineren Teil und relativ spät in die rheumatologische Versorgung. Rheumatologen unterscheiden sich in dem Spektrum der von ihnen verordneten Therapien systematisch von Hausärzten. Dies zeigt sich vor allem am Versorgungsgrad mit Basistherapie. Aber auch ergänzende Therapien, Hilfsmittel und Rehabilitationsmaßnahmen werden in rheumatologischer Behandlung häufiger veranlaßt. Die intensive Betreuung findet ihren positiven Niederschlag in größerer Eigenaktivität der Patienten und einer besseren Beurteilung des subjektiven Gesundheitszustandes. Dauerhaft rheumatologisch betreute Patienten zeigen im Zwei-Jahres-Längsschnitt mit einem mittleren Funktionsverlust von knapp vier Prozentpunkten eine eher günstige Entwicklung des Funktionsstatus.
