Azathioprin-Ingestion in suizidaler Absicht bei einem Jugendlichen mit juveniler chronischer Polyarthritis

Abstract

We report on a 13 years 7 months old boy who ingested 650 mg azathioprine in a suicide attempt. His baseline medication was azathioprine and methotrexate for control of juvenile chronic polyarthritis. After the induction of vomiting by ipecacuanha sirup and administration of charcoal (1 g/kg), he was closely followed for haematological, hepatic, and renal side effects. During the following days, no serious adverse events were noted except that the thrombocyte (from 403 000 down to 199000/µ.l) and total leukocyte count decreased moderately (from 12000 down to 7100/µl). On the basis of this case report and the available literature, the potential acute toxicity of azathioprine and possible treatment modalities are discussed.

Zusammenfassung

Wir berichten über einen 13 Jahre, 7 Monate alten Jugendlichen, der in suizidaler Absicht 650 mg Azathioprin einnahm. Er wurde wegen einer juvenilen chronischen Polyarthritis mit Azathioprin und Methotrexat behandelt. Nach dem Versuch der primären Giftentfernung mittels induzierten Erbrechens und der anschließenden Gabe von Kohle (1 g/kg KG) wurde er bezüglich hämatopoetischer, hepatischer und renaler Nebenwirkungen engmaschig überwacht. Im weiteren Verlauf traten jedoch außer einem mäßigen Abfall der Thrombozyten (von 403 000 auf 199000/µl und der Leukozyten (von 12000 auf 7100/µl) keine Nebenwirkungen auf. Anhand dieses Falles und der Literatur werden die potentielle Akuttoxizität von Azathioprin und mögliche therapeutische Optionen diskutiert.
