Wilms' Tumour in a Horseshoe Kidney. Diagnostic Difficulties

Abstract

A case of Wilms' tumour in a horseshoe kidney is presented. Until 1980, twenty-one cases were reported. Diagnosis may be difficult, especially as x-ray investigations, including IVP and renal angiograms, give the impression of a tumour arising outside the kidney. The distortion and dispersion of the renal pelvis and calyces usually seen with a Wilms' tumour are lacking. This is probably due to the special anatomic pattern of the horseshoe kidney, specifically the rotation of the renal pelvis around the long axis of the kidney.

Zusammenfassung

Es wird ein Fall von Wilms-Tumor in einer Hufeisenniere vorgestellt. Bis 1980 war über 21 solche Fälle berichtet worden. Die Diagnose kann besonders dann schwierig sein, wenn die Röntgenuntersuchungen (IVP und Renovasogramm) den Eindruck ergeben, daß es sich um einen Tumor handelt, der außerhalb der Niere entstanden ist. Die Drehung und Ausziehung des Nierenbeckens und der Kelche, die gewöhnlich bei einem Wilms-Tumor zu beobachten sind, fehlen. Das ist wahrscheinlich durch die besonderen anatomischen Verhältnisse bei einer Hufeisenniere, besonders durch Drehung des Nierenbeckens um die Längsachse der Niere bedingt.
