The Duhamel/Martin Operation in the Treatment of Hirschsprung's Disease

Abstract

Thirty-four patients with Hirschsprung's disease, 17 of whom were operated on by the original Duhamel procedure and 17 by the Martin modification are reported on. Thirty-two have had a satisfactory result. Fourteen patients developed early complications. One patient died. Twenty-two have been followed for more than five years. The Martin modification obviates the main disadvantage of the Duhamel operation. A proximal temporary diversionary colostomy or ileostomy greatly reduces the incidence of complications following the pull-through procedures.

Zusammenfassung

Es wird über 34 Patienten mit einem Morbus Hirschsprung berichtet. 17 der Kinder waren nach Duhamel, 17 weitere nach der Modifikation von Lester Martin operiert worden. 32 der 34 Patienten zeigten später ein zufriedenstellendes Ergebnis. 14 Kinder entwickelten Frühkomplikationen, ein Patient starb.

22 Kinder konnten mehr als fünf Jahre postoperativ nachuntersucht werden. Die Modifikation nach Martin vermeidet die Nachteile des Original-Duhamelschen-Verfahrens. Eine proximale vorübergehende Kolostomie oder Ileostomie reduziert dabei die Häufigkeit von Komplikationen nach dem retrorektalen Durchzug.
