Staphylokokken-Endophthalmitis nach Schieloperation

Zusammenfassung

Drei Tage nach simultan rechts und links extern durchgeführter Schieloperation entwickelte sich bei einem 10jährigen Jungen eine akute purulente Endophthalmitis des rechten Auges mit einem Visus von LSiP. Nach der bakteriellen und mykotischen Abstrichdiagnostik wurde eine “ad hoc-Antibiose” mit Cephalosporinen und Amikacin in Kombination mit immunsup-pressiven Dosen von Methylprednisolon systemisch durchgeführt. 9 Tage nach der Schieloperation erfolgte eine Pars plana-Vitrektomie. Sowohl im Wundsekret wie im Glaskörperaspirat ließen sich Koagulase-negati-ve, grampositive Kokken identifizieren. Wegen einer totalen Traktionsamotio retinae mußte 30 Tage nach der Schieloperation eine Silikonölinstillation und Phakek-tomie durchgeführt werden. Visus 3 Monate nach Schieloperation: R: cc 0.2.

Summary

A 10-year-old boy was referred to the authors three days after bimedial faden operation (posterior fixation), with a purulent endophthalmitis in his right eye and visual acuity of LP. Systemic combined antibiotics “ad hoc”, including cephalosporins and amikacin, were administered in combination with immunosuppressive doses of methylprednisolone after swabs had been taken for bactériologie workup. Nine days after admission a pars plana vitrectomy was performed. Both the conjunctival swabs and the vitreous aspirate grew coagulase-negative, gram-positive cocci with high colony counts. Traction retinal detachment developed, and 30 days after the squint surgery lensecto-my and silicone oil instillation had to be performed. Visual acuity 3 months after squint surgery was RE cc 0.2.
