Abstract
154 children aged 2 to 12 years with clinical diagnosis of bacterial pharyngitis and/or tonsillitis and - in most of the patients - a positive enzyme immunoassay for group A β-hemolytic streptococci before therapy were enrolled in this open controlled randomized and multicenter trial.
The children received either 8 mg/kg bodyweight cefixime once daily or 20000 I.E. pencilline V/kg bodyweight t.i.d. Clinical evaluation and microbiological tests were carried out before treatment and 1-5 days after end of the treatment. 3-4 weeks after end of the treatment the rate of relapses was evaluated.
The data of 149 children could be evaluated for clinical efficacy. In the cefixime group 93,3% of the children were cured and 6,7% improved compared to 89,2% and 10,8%, respectively, in the penicillin V group.
Complete microbiological data were obtained from 136 patients. The eradication rate was 82,7% in the cefixime group and 77% in the group of patients treated with penicillin V. At follow up relapses were seen in 7 of the cefixime treated patients and in 6 of those receiving penicillin V.
Mild side effects were reported by 4 patients in the cefixime group and by 3 children treated with penicillin V (1 drop out each).
These resultes show that cefixime once daily is at least as effective as penicillin V t.i.d. in pharyngitis and tonsillitis in children. Both compounds are well tolerated.
Zusammenfassung
154 Kinder im Alter von 2 bis 12 Jahren mit klinischen Symptomen einer bakteriellen Pharyngitis und/oder Tonsillitis nahmen an dieser offenen, kontrollierten und randomisierten Multicenterstudie teil. Die meisten Patienten hatten vor Studienbeginn einen positiven Schnelltest auf β-hämolysierende Streptokokken der Gruppe A.
Die Kinder erhielten entweder 8 mg/kg Körpergewicht Cefixim 1× täglich oder 3× täglich 20000 I.E. Penicillin V/kg Körpergewicht. Mikrobiologische Tests und körperliche Untersuchungen wurden vor Therapie und 1-5 Tage nach Therapieende durchgeführt. 3-4 Wochen nach Behandlungsende wurde die Rezidivrate ermittelt.
Die Daten von 149 Kindern konnten zur klinischen Wirksamkeit ausgewertet werden. In der Cefixim-Gruppe wurden 93,3% geheilt und bei 6,7% der Kinder wurden die Symptome deutlich gebessert, verglichen mit 89,2% und 10,8% in der Penicillin V-Gruppe.
Mikrobiologische Daten lagen von 136 Kindern vor. Die Eradikationsrate betrug in der Cefixim-Gruppe 82,7% und in der Penicillin-V-Gruppe 77%. 3-4 Wochen nach Therapieende wurden 7 Rezidive in der Cefixim-Gruppe und 6 Rezidive in der Penicillin V-Gruppe festgestellt.
Leichte Nebenwirkungen wurden von 4 Kindern in der Cefixim-Gruppe und 3 Kindern in der Penicillin V-Gruppe angegeben. In je 1 Fall mußte die Behandlung abgebrochen werden.
Die Ergebnisse dieser Studie zeigen, daß bei bakterieller Pharyngitis und Tonsillitis die 1 × tägliche Gabe von Cefixim mindestens ebenso gut wirksam ist wie die 3× tägliche Gabe von Penicillin V. Beide Antibiotika werden gut vertragen.
