Abstract
Rheumatoid arthritis affects the foot as frequently as the hand and wrist. In addition to destructive changes, the disease causes growth disturbances in children, often asymmetrie, and in itself leading to severe functional disturbances. Early synovectomy may lead to correction of growth disturbances. The technique of synovextomy in the talocrural, subtalar and the tarsal joints as well as the toe joints is described and the results 5,7 years after synovectomy of the talocrural and tarsal joints in 53 patients are reported.
Whereas early (prophylactic) synovectomy is much recommended, reconstructive surgery is still done more frequently. The technique of arthroplasty and other reconstructions of toe joints, subtalar and talocrural joints is described. Arthrodesis of the talocrural joint in rheumatoid arthritis is not recommendable as the neighbouring joints are usually affected, and has not been performed in our clinie since 1976. The technique of replacement arthroplasty of the talocrural joint and results of 66 arthroplasties performed between 1975 und 1984 are reported. The overall results regarding postoperative pain and range of motion are satisfactory. The small number of loosenings, in contrast to results reported from other clinics, is probably explainable by the use of special guiding instruments during operation, the low body-weight and the reduced muscle power found in polyarthritic patients. It is felt that the side flanges of the tibial prosthesis of the Thompson/Parkridge/Richards ankle prosthesis may have played an important role in a side stability. Tripie arthrodesis is a beneficial procedure and should be done prior to talocrural arthroplasty if needed.
Zusammenfassung
Der Fuß ist bei der chronischen Polyarthritis ebenso häufig befallen wie die Hand und das Handgelenk. Die Erkrankung bewirkt zusätzlich zu destruktiven Veränderungen bei Kindern Wachstumsstörungen, die oft asymmetrisch auftreten und ihrerseits zu schweren funktionellen Behinderungen führen. Frühsynovektomien können diese Wachstumsstörungen korrigierend beeinflussen.
Im einzelnen werden die Techniken der Synovektomie, der Arthroplastik und anderer rekonstruktiver Maßnahmen für die talokruralen, subtalaren und tarsalen Gelenke sowie für die Zehengelenke beschrieben.
Die Ergebnisse 5, 7 Jahre nach Synovektomie der Talokrural- und Tarsalgelenke bei 53 Patienten werden dargestellt. Zwar wird die Frühsynovektomie als prophylaktischer Eingriff bevorzugt, jedoch wurden häufiger rekonstruktive Maßnahmen durchgeführt. Eine Arthrodese des oberen Sprunggelenkes ist bei chronischer Polyarthritis wegen des Mitbefalls der Nachbargelenke nicht zu empfehlen, daher wurde eine solche Operation bei uns seit 1976 nicht mehr vorgenommen.
Nach Darstellung der speziellen Implantationstechnik wird über die Ergebnisse von 66 TPR-Prothesen Thompson-Parkridge-Richards am oberen Sprunggelenk berichtet, die zwischen 1975 und 1984 implantiert worden sind. Die Ergebnisse sind hinsichtlich der Schmerzlinderung und Verbesserung der Beweglichkeit zufriedenstelIend. Die geringe Zahl der Lockerung, insbesondere im Vergleich zu Berichten anderer Kliniken, ist wohl erklärbar durch die Verwendung spezieller Zielgeräte bei der Operation sowie durch das geringe Körpergewicht und die verminderte Muskelkraft der Rheumatiker. Es scheint, daß insbesondere durch die Seitenflanschen an der tibialen Prothesenkomponente bei der TPR-Sprunggelenkprothese ein wichtiger Faktor zur Sicherung der Seitstabilität gegeben ist. Die Tripelarthrodese ist eine wertvolle Operation, die falls erforderlich, vor Implatantion der Sprunggelenkprothese durchgeführt werden sollte.
