Abstract
Treatment of chronic rheumatic diseases can be orientated only according to the signs and symptoms of pain, inflammation and restricted mobility - which is quite logical, since we still lack knowledge on the aetiology and pathogenesis of most rheumatic diseases. For this reason, treatment is of course multifarious and varied. The focus is on drug treatment, physiotherapy and surgery. Since it is necessary to coordinate such measures meaningfully to result in a therapy plan, it is therefore imperative to arrange for interdisciplinary cooperation. The main tasks of such interdisciplinary cooperation between rheumatologists active in conservative therapy and in surgery, are to establish indications for surgery and to arrange for preoperative and postoperative care of the patient suffering from rheumatic disease.
The "Rheumatism Unit" of the Ludwig Maximilians University at Munich is one form of organisation of interdisciplinary cooperation between internist rheumatologists and orthopaedists active in surgery of rheumatic patients. This interdisciplinary institution is the logical further development and institutionalisation of a cooperation that has been in the process of realisation during the past 20 years as a result of clinical necessity.
An institution enabling optimal care of rheumatic patients is at the same time suited for research work in rheumatology provided structural conditions are adequate.
Zusammenfassung
Die Behandlung chronischer rheumatischer Krankheiten kann sich aufgrund heute noch fehlender Kenntnisse über Ätiologie und Pathogenese der meisten rheumatischen Krankheiten folgerichtig nur an den Krankheitsäußerungen Schmerz, Entzündung und Bewegungseinschränkung orientieren. Sie ist daher vielfältig. Medikamentöse, physikalisch-medizinische und operative Maßnahmen stehen im Mittelpunkt. Die Notwendigkeit des sinnvollen Zusammenfügens der einzelnen Maßnahmen zu einem Therapieplan begründet auch die Notwendigkeit einer interdisziplinären Zusammenarbeit.
Die Hauptaufgaben einer solchen interdisziplinären Zusammenarbeit zwischen konservativen und operativ tätigen Rheumatologen sind die Erarbeitung der Indikationen zum chirurgischen Vorgehen und die prä- und postoperative Betreuung des Rheumapatienten.
Eine Organisationsform einer interdisziplinären Kooperation zwischen internistischen Rheumatologen und rheuma-chirurgisch tätigen Orthopäden ist die Rheuma-Einheit der Ludwig-Maximilians-Universität München. Diese interdisziplinäre Einrichtung ist die konsequente Weiterentwicklung und Institutionalisierung einer in 20 Jahren aus der klinischen Notwendigkeit heraus entstandenen Zusammenarbeit.
Eine Einrichtung, welche eine optimale Versorgung Rheumakranker ermöglicht, ist gleichzeitig unter entsprechenden strukturellen Bedingungen auch geeignet für die wissenschaftliche Arbeit auf dem Gebiet der Rheumatologie.
